What the inspector wrote
On 9/18/2024, Licensing Program Analysts (LPAs) Tatiana Bickham and Amelia Morales conducted an announced pre-licensing inspection. The purpose of the inspection was to ensure that the health, safety, and personal rights as required by Title 22 Regulations governing California Family Child Care Homes will be met by the applicant. LPAs were greeted and let into the residence by Applicant Anton Skypnyk and Larysa Shevchuk, who guided LPAs on a tour of the inside and outside of the residence. An entrance checklist was provided. This is a change of location; previous licensing number was 197494680. Applicants understand once the new residence is a licensed, care can no longer be provided at the other address

The applicants are requesting a Large capacity of 14 children ages 1 to 6. The Family Child Care Home will operate Monday to Friday from 8:00 am to 6:00 pm.
The applicants completed Preventive Health and Safety/Childhood Nutrition/ Lead Exposure Prevention on 08/23/2024 and Mandated Reporter training on 07/02/2024 and 7/15/2024. The applicant has current Pediatric CPR/First Aid training completed on 07/20/2024; the trainer was EMSA certified.
The inspection begun at 8:10 am. Upon arrival there is an entry gate that parents will need to walk through that has a latch at the top of the gate. This residence is a two story home consisting of 3 bedrooms and 2 ½ bathrooms. The areas that will be accessible to the children in care are: living room/playroom, garage/playroom, the kitchen, restroom #1 on the first floor, attached garage, backyard, and front yard both are fenced around the perimeter.
LPAs informed applicant the converted garage/playroom can only be used for activities and cannot be used for the children to sleep, eat, or do any toileting. Applicants understand that they can only use the garage/playroom for activities.

These areas will be off-limits to the children in care: laundry room located inside the garage/playroom, the second floor, the balcony located on the second floor, the 3 bedrooms upstairs and bathroom #2, and part of the backyard. The second floor is rendered inaccessible to the children in care by a baby gate which is installed in front of the stairwell.
In the kitchen, cleaning supplies are stored in the cabinet underneath the sink; it is rendered inaccessible by a child proof latch. The knives are stored in a locked drawer, rendering it inaccessible to the children in care.
LPAs observed the following: the home is clean and orderly. There were ag

appropriate toys and play items.
Applicants were informed that baby-walkers, bouncers, jumpers, and other prohibited items will not be used for children in care.
All electrical outlets have safety covers.
There is a combination smoke/carbon monoxide detector in the ceiling of garage/playroom as well as inside the laundry room, the second floor hallway, stairwell. All three detectors were tested and found to be operable. The required fire extinguisher (size 2A:10-B:C) was observed in the laundry room; it was purchased in September 2024. There were detergents, cleaning compounds, in bathroom #1. They were rendered inaccessible to children by a safety latch in a top shelf. In the living room, there is a fireplace which is screened. For napping equipment, there are 12 cots and one play yard. Per applicants they will provide fitted sheets that will be washed weekly.
Per applicants, there are no weapons or firearms of any kind in the facility currently. LPAs discussed that when a child shows signs of illness, they shall be separated from other children until they are picked up. The isolation area will be in the garage/play area, separate from other children in care. Per applicants, the children will have access to part of the backyard, and full access to the front yard that are fenced off. The off-limits area of the backyard is separated by a gate. Per applicants, when children are having outside time, they will ensure 100% supervision and never leave children unattended.

There are no bodies of water on the premise.
LPAs reviewed with applicant form LIC 311D Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. LPAs provided applicant the following required postings: LIC 610A Emergency Disaster Plan, LIC 9148 Earthquake Preparedness Checklist, and PUB394 Notification of Parents Rights Poster. LPAs advised that once licensed, the facility license shall be posted. A fire/disaster drill log shall be available and performed at least every six months.
Applicants were reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
The applicants, rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).

LPAs discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at:

, as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at:

, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPAs provided applicants with a sample sleep log and the sleeping log.
On this date, 08/06/2024, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
Applicants were informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
Exit interview conducted and report was reviewed with Applicant Anton Skypnyk. Due to prior engagement applicant Larysa Shevchuk was unable to conduct the exit interview with LPAs. Applicants will be submitted for approval for a Large Family Child Care with a capacity of 14 Children.
