On 11/5/2025 Licensing Program Analyst (LPA) Jeanine Lipsey conducted an continued unannounced, case management incident inspection.\302\240 Upon arrival, LPA met with and were guided on a tour of the facility by Office Manager Julio Pelayo, to whom the purpose of the inspection was announced. Census was 81 children and 16 staff. Room 1 children 17 staff 3 Room 2 children 20 staff 3 Room 3 children 16 staff 6 Room 4 children 15 staff 2 Room 5 children 13 staff 2 The incident was reported to Community Care Licensing on 10/6/25 via phone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. The incident which occurred on or before 10/3/25 was that a substitute staff member hit a child on the back and face. During the inspection, LPA interviewed 4 children. This incident needs further information. Exit interview conducted and report was reviewed with Lead Teacher Alison Choi. A Notice of Site Visit was given and must remain posted for 30 days
