What the inspector wrote

On 05/08/2025 Licensing Program Analysts (LPA) Laticia Thompson met with Scott Bartholomew Applicant. The purpose of the visit was an announced pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. This is an application for a School \342\200\223 Age license for a total capacity of 85 children ages 5-12 years of age. The child care center will operate Monday, Wednesday, Thursday and Friday 2:30PM \342\200\223 6:00 PM. Tuesday 1:30PM - 6:00 PM. The facility is located on the campus of Welby Way Charter School which is a part of the Los Angeles Unified School District. There is an approved fire clearance. The facility has usage of 3 classrooms, Room 41 and Room 42. LPA toured and measured the on-limits area s. Per Health and Safety Code Sections the following exemptions shall apply: 1596.806(a), (a)(1), (a)(2) state: (a) A room used as a classroom by a school-age child care program shall not be required to meet the square footage or toilet requirements for child day care centers if the program is operated on either of the following School-age child care programs that meet the requirements of Health and Safety Code Section 1596.806 are exempt from square-footage requirements for outdoor activity space . The Indoor total measurements contain a total of 1,334.4 based on today\342\200\231s measurement the facility capacity limit for indoor space is 38 children. Parents will sign in, sign out manually and enter and exit through the front gate of the elementary school. The facility has filtered water fountains where children can fill their personal water bottles for outdoor and indoor activities. The facility does not have an operable carbon monoxide detector. vLPA observed fire extinguishers with an inspection date of 11/12/2024. The facility does not have First Aid Kits. Furniture and equipment were observed to be maintained in good condition free of sharp, loose or pointed parts. Tables and chairs were observed to meet the needs of the children. The facility does not have a place for storage of children clothing and personal belongings. The classrooms have central air for heating and cooling. The classrooms were not clean. Classrooms have adequate lighting. Windows are in need of removal of dust. LPA did not observe trash cans with tight fitting lids. The facility plans to be equipped with a cell phone designated specifically for classroom usage. Disinfectants, cleaning solutions, and other items that are dangerous to children will be stored under the sink with a safety lock inaccessible to children. All toilets and hand washing facilities are maintained in safe and sanitary operating condition. The applicant shall be responsible for ensuring that children with obvious symptoms of illness are not accepted. The isolation area for Ill children will be located in the nurses office and restroom . The applicant was advised to develop and implement a written plan to record the administration of prescription and nonprescription medications, to inform the child's authorized representative daily when such medications have been given. Medication will be stored in a locked box located in classroom #42 inaccessible to children. Medication that requires refrigeration will be stored in the refrigerator located in classroom #42 inaccessible to children. The program will provide snacks through the Beyond the Bell program. Applicant was reminded that menus shall be in writing and posted in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request. The required postings will be posted in the front of the school at the front gate for parents and visitors to review. Facility has a disaster and mass casualty plan of action. The plan will be in writing and readily available with contingency plans for action during fires, floods and earthquakes. The facility does not provide transportation. Firearms and other weapons shall not be allowed on or stored on the premises of a child care. Applicant stated there aren\342\200\231t any firearms or weapons on the premises. Smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). Licensees shall reveal each child care center license number in all advertisements in accordance with Health and Safety Code Section 1596.861 The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The outdoor activity space was observed and maintained in good condition. LPA did not observe any bodies of water. The following items were discussed: The director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. LPA reviewed with facility representative the LIC 311A, Records To Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. The following corrections are needed before a licensure will be co nsidered. Facility will install a carbon monoxide detector Facility will ensure floor surface and rugs are cleaned and in sanitary condition. Facility will provide pictures of cubbies where children will store personal belongings. Facility will provide pictures of first aid kit Facility will ensure facility is clean, safe and in sanitary to ensure the safety and well being of children by removing/relocating storage containers.Facility will provide pictures of Child Passenger restraint system posting Facility will install trash can with tight fitting lids. Facility will provide a picture of thermostat inside classroom refrigerator. Facility will provide pictures of require postings. Follow up with Fire Department to confirm classes approved Control of Property Corrections are due by 06/20/2025 An exit interview was conducted with plan of corrected items discussed. LPA provided Applicant with appeal rights and a copy of this report. Final license determination will be made upon review by the Licensing Program Manager and corrections.
