Inspector’s narrative

What the inspector wrote

On 9/5/25 LPA observed drop off time. At 8:00AM Staff 1 was alone with 5 infant children. Staff 3, who is usually in the infant room, was preparing breakfast for the whole facility in the kitchen area outside the office. At 8:04AM Staff 5 accepted a 13th preschool aged child into care. At 8:07AM Staff 5 accepted an infant child into care. Staff 5 was alone with 13 preschoolers and 1 infant child. At 8:23AM Staff 1 was alone with 8 infant children. Staff 3 was delivering breakfast to the classrooms. Based on LPAs observations and interviews which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 Chapter 1 is being cited on the attached LIC 9099D. A notice of site visit was given and must remain posted for 30 days. Appeal Rights provided. Exit interview conducted and report was reviewed with Licensee Anna Baghramyan. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. A notice of site visit was given and must remain posted for 30 days. Appeal Rights provided. Exit interview conducted and report was reviewed with Licensee Anna Baghramyan.

  • TEACHER-CHILD RATIO

    (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below.This requirement was not met as evidenced by: Based on observation on 9/5/25, Staff 5 (S5) had 13 preschool children and one infant child. This poses an immediate health, safety, or personal rights risk to persons in care.

  • FOOD SERVICE

    (a) In child care centers providing meals to children, the following shall apply: (1) ...Each meal shall include, at a minimum, the amount of food components as specified by...Requirements for Meals, for the age group served. This requirement is not met as evidenced by: Based on record review of menus and reciepe book, and observations, facility does not offer the amount of food components as specified in the regulations. This poses a potential health, safety, or personal rights risk to persons in care.
