What the inspector wrote

THIS REPORT HAS BEEN AMENDED TO REFLECT THE FOLLOWING CHANGES PRESCHOOL INDOOR MEASUREMENTS WILL ACCOMMODATE 52 CHILDREN. On 12/30/2025 Licensing Program Analysts (LPA\342\200\231s) Laticia Thompson and Janet Gil met with Kristine Kostandyan. Applicant. The purpose of the visit was an announced pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were no children present during this inspection. This is an application for a Single License with a Total capacity of 60; 25 Toddlers, 25 Preschoolers. ages 2 years into entry of 1 st grade and 10 School Age Children. During today\342\200\231s visit Applicant requested to change capacity to 53 preschoolers based on facility measurement. The child care center will operate Monday \342\200\223 Friday 8:00 a.m. \342\200\223 5:00 p.m. LPA\342\200\231s toured and measured the on-limits areas. The fire clearance was approved on 07/24/2025. The facility has a total of 5 classrooms. Preschool children will have access to 5 sinks and 5 toilets. Preschool Indoor measurements are : The measurements contain a total of 1,844.35 square feet which will accommodate 52 children. Preschool Outdoor Activity measurements are: The outdoor activity space measurements contain a total of 4,660.38 square feet which will accommodate 52 Preschool children. The following was observed: Parents will enter and exit through a secured gate that requires the facility to allow access through a buzzer. Parents will sign children in and out electronically under the ProCare application. LPA advised facility there should be a plan in place on how such records will be immediately printable upon request for inspection and audit by program staff. For best practice purposes, facilities should have a plan in place on how records would be backed up, and how they would be made available in the event of a power failure or disaster. LPA reference director to review PIN 18-03.1-CCP. Applicant stated that if currently enrolled children parents prefer to continue signing in and out manually. This will take place in the directors office on a sign in and out sheet. The staff restroom is located next to room #3. The facility has a filtered water dispensers where staff will fill children personal water bottles for outdoor and indoor activities. In the future Applicant will install a filtered water system in the kitchen sink. Applicant was advised to notify department prior to changes being made. There are 2 operable carbon monoxide detector located in classrooms 3 and 4. LPA observed fire extinguishers with an inspection date of 03/11/2025. LPA observed First Aid Kits with the required materials. Furniture and equipment were observed to be maintained in good condition free of sharp, loose or pointed parts. Tables and chairs were observed to meet the needs of the children. There are cots and mats for napping maintained in a safe condition. LPA confirmed that mats or colored so that the sleeping side red can be distinguished from the floor side blue. There is sufficient quantity and variety of age- appropriate play equipment in good condition. There are cubbies in all classrooms for storage of children clothing and personal belongings. Classrooms 1, 2, and 3 have central air for heating and cooling. Classroom 4 and 5 have portable heaters and air condition wall units. The classrooms were clean in good repair. Classrooms have adequate lighting. Windows were in good repair free of chipping paint, dirt, insects, or debris. The outside windows located on the side yard window are in repair of chipping paint. Facility will install trash cans with lids. The facility will be equipped with a cellphone that will remain in the facility during operating hours. Disinfectants, cleaning solutions, and other items that are dangerous to children are stored in the kitchen cabinets inaccessible to children. All toilets and handwashing facilities are maintained in safe and sanitary operating condition. Handwashing fixtures do not deliver hot water. The applicant shall be responsible for ensuring that children with obvious symptoms of illness are not accepted. The isolation area for Ill children will be located in the directors office and staff restroom. The isolation area shall be equipped with a cot for ill children and shall be supervised by staff. The applicant was advised to develop and implement a written plan to record the administration of prescription and nonprescription medications, to inform the child's authorized representative daily when such medications have been given. Medication will be stored in the storage closet in the directors office inaccessible to children. Medication that requires refrigeration will be stored in the refrigerator located in the kitchen inaccessible to children. The program provides snacks. The kitchen and food-preparation storage areas were observed to be clean and free of litter and rubbish. All foods or beverages that require storage at a temperature of 45 degrees will be stored in the refrigerator located in the kitchen. The refrigerator is equipped with a thermostat to regulate temperature control. Applicant stated refrigerator will be replace with a new version. Menus are in writing and are posted in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request. The required postings are posted on a wall next to room 1 and 2 entrance, for parents and visitors to review. Facility does not have a disaster and mass casualty plan of action. The plan shall be in writing and shall be readily available with contingency plans for action during fires, floods and earthquakes. The facility does not provide transportation. Firearms and other weapons shall not be allowed on or stored on the premises of a child care. The applicant stated there aren\342\200\231t any firearms or weapons on the premises. Smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). Licensees shall reveal each child care center license number in all advertisements in accordance with Health and Safety Code Section 1596.861 Outdoor The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. Play equipment and materials used by children need to be evaluated for age-appropriateness. The fence is in need of some repair in the front and side outdoor activity space. Areas around and under climbing equipment, swings, slides and other similar equipment has sand. Applicant plans to install artificial grass for cushioning to prevent injury from falls. The outdoor activity space was observed and maintained in good condition LPA observed multiple hazards and concerns in the outdoor activity area (see plans of corrections). There is adequate shade and benches for resting in the outdoor activity area. Applicant was advised to inspect sand grounds daily to ensure areas are free of any hazardous foreign material. LPA did not observe any bodies of water. The following items were discussed: The director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA reviewed with applicant the LIC 311A, Records to be Maintained at the Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Entrance Checklist was provided to the applicant. LPA discussed the safe sleep regulations with [applicant, licensee, or facility representative] and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep , as an additional resource. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for and removing recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters, and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication. The following corrections are needed before licensure will be considered. FACILITY WILL PROVIDE A DISASTER MASS CASUALTY PLAN AND AN UPDATED FACILITY SKETCH. APPLICANT WILL SHAMPOO RUGS IN CLASSROOMS APPLICANT WILL ENSURE THAT ALL TRASH CANS HAVE LIDS APPLICANT WILL PROVIDE PICTURES OF BARRICADE IN FRONT OF WALL HEATER LOCATED IN ROOM 4 AND 5. PROVIDE PROOF OF AGE REGULATIONS FOR PLAY EQUIPMENT LOCATED IN FRONT YARD. REPAIR CHIPPING PAINT ON PLAY EQUIPMENT LOCATED IN THE BACK YARD PLAY AREA. APPLICANT WILL PLACE CUSHIONING AROUND EXPOSE PIPES AND AIR CONDITIONING UNITS. REPAIR CHIPPING PAINT ON OUTSIDE WINDOW IN THE SIDE PLAY AREA. REPAIR OR REPLACE LINK CHAIN GATES IN FRONT AND SIDE YARD AREA. APPLICANT WILL ENSURE THAT BIKE APPARATUS EQUIPMENT IS ANCHORED AND LEVELED CORRECTLY. Corrections are due by 01/27/2026 An exit interview was conducted with plan of corrected items discussed. LPA provided Kristine Kostandyan with appeal rights and a copy of this report. Final license determination will be made upon review by the Licensing Program Manager and corrections.
