What the inspector wrote

On 10/9/2025 Licensing Program Analysts (LPA) Laticia Thompson and Licensing Program Manager (LPM) Deborah Lowe met with Lisette Sepulveda, Principal. The purpose of the visit was an announced pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were no children present during this inspection. This is an application for a License with a Total capacity of 72 Preschoolers ages 2 years into entry of 1 st grade. The child care center will operate Monday \342\200\223 Friday 7:00 a.m. \342\200\223 5:30 p.m. LPA toured and measured the on-limits areas. The fire clearance was approved on 09/29/2025. The facility has 1 classroom which is a large open area. Preschool children will have access to 4 sinks and 5 toilets. Preschool Indoor measurements are: The measurements contain a total of 2,268.14 which will not accommodate Applicant\342\200\231s request for 72 Preschool children. Based on today\342\200\231s measurement the facility capacity limit for indoor space is 60 Preschoolers Preschool Outdoor Activity measurements are: The outdoor activity space measurements contain a total of 4,715. 58 which will not accommodate Applicant\342\200\231s request for 72 Preschool children. Based on today\342\200\231s measurement the facility capacity limit for outdoor space is 60 Preschool children Furniture and equipment were observed to be maintained in good condition free of sharp, loose or pointed parts. Tables and chairs were observed to meet the needs of the children. There are cots for napping. There are cubbies for storage of children clothing and personal belongings. The classrooms have central air for heating and cooling. The classrooms were clean in good repair. Classrooms have adequate lighting. Windows were in good repair free of chipping paint, dirt, insects, or debris. LPA observed trash cans with tight fitting lids. The facility is equipped with landline telephones service. Disinfectants, cleaning solutions, and other items that are dangerous to children are inaccessible and stored in the locked storage room next to the staff lounge area. All toilets and hand washing facilities are maintained in safe and sanitary operating conditions. Water fixtures do not deliver hot water. The applicant shall be responsible for ensuring that children with obvious symptoms of illness are not accepted. The isolation area for Ill children will be located in the principal\342\200\231s office and the staff restroom. The isolation area shall be equipped with a cot for ill children and shall be supervised by staff. The applicant has developed a written plan to record the administration of prescription and nonprescription medications, to inform the child's authorized representative daily when such medications have been given. Medication will be stored in the staff lounge inaccessible to children. Medication that requires refrigeration will be stored in the staff refrigerator inaccessible to children. The program provides breakfast lunch and snacks. Food will be delivered and prepped in an alternate school district location. Menus are in writing and are posted in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request. The required postings are posted at the front entrance for parents and visitors to review. Facility has a disaster and mass casualty plan of action. LPA advised Director that the plan shall be in writing and be readily available with contingency plans for action during fires, floods and earthquakes. The facility does not provide transportation. The principal stated firearms and other weapons shall not be allowed on or stored on the premises of the child care facility . Smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). Licensees shall reveal each child care center license number in all advertisements in accordance with Health and Safety Code Section 1596.861 Parents will enter and exit through the front entrance located near the Victory Blvd parking lot. Parents will sign children in and out manually on the sign in and out sheets. Staff restrooms are located near front entrance of the facility. The facility has 2 water fountains located in the indoor and outdoor activity space. There is an operable carbon monoxide detector. LPA observed fire extinguishers with an inspection date of 07/28/2025. LPA observed First Aid Kits with the required materials including disposable thermometer strips. Outdoor The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. The climbing equipment slides or other similar equipment has a rubber platform to prevent injuries. The outdoor activity space was observed and maintained in good condition including age appropriate play equipment. LPA did not observe any equipment or hazards in the outdoor activity area. The principal ensured the gate that leads to the occupational center will be quipped with a padded lock. There is adequate shade and benches for resting in the outdoor activity area. LPA did not observe any bodies of water. Facility representative confirmed there are no bodies of water on the premises. The following items were discussed: The director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. LPA reviewed with facility representative the LIC 311A, Records To Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). An exit interview was conducted and the report was reviewed with Martha Borquez facility representative. Principal Lisette Sepulveda was unavailable due to a training. Facility Representative was provided with appeal rights and a copy of this report. Final license determination will be made upon review by the Licensing Program Manager .
