What the inspector wrote

On 12/23/2025 Licensing Program Analysts (LPA) Laticia Thompson and Licensing Program Manager (LPM) Deborah Lowe met with Ann Jorden (Liz), Applicant and Sandy Trigo, Director. The purpose of the visit was an announced pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were no children present during this inspection. This is an application for a Single License with a requested Total capacity of 55; 12 Toddlers ages 18 \342\200\223 36 months and 43 Preschoolers. ages 2 years into entry of 1 st grade. The child care center will operate Monday \342\200\223 Thursday 8:00 a.m. \342\200\223 4:30 p.m and Fridays 8:00am \342\200\223 3:30pm LPA toured and measured the on-limits areas. The fire clearance was approved on 10/23/2025. The facility has a total of 5 classrooms. The Red classroom will be dedicated to the Toddler program and classrooms, Green, Orange, Butterfly and Activity rooms are dedicated to the preschool. Preschool children will have access to 4 sinks, 5 toilets and 2 urinals. Toddlers will have access to 1 sink and 1 toilet. Toddler Indoor measurements are : The measurements contain a total of 374.55 square feet which will not accommodate Applicant\342\200\231s request for 12 Toddlers. Based on today\342\200\231s measurement the facility capacity limit for indoor space is 11 Toddlers Preschool Indoor measurements are: The measurements contain a total of 1,487.21 square feet which will not accommodate Applicant\342\200\231s request for 43 Preschool children. Based on today\342\200\231s measurement the facility capacity limit for indoor space is 42 Preschoolers Toddler Outdoor Activity measurements are: The outdoor activity space measurements contain a total of 2,337.49 square feet which will accommodate Applicant\342\200\231s request for toddlers Preschool Outdoor Activity measurements are: The outdoor activity space measurements contain a total of 6,338 square feet which will accommodate Applicant\342\200\231s request for Preschool children. The following was observed: The facility is equipped with surveillance cameras with a view of outdoor activity space only. Parents will enter and exit on Sale Street through a secure entrance door. Parents will sign children in and out manually on sheets located at the entrance of the activity room. The Staff restroom is located next to the orange room. The facility has a filtered water dispenser in the kitchen. Staff will fill children personal water bottles for outdoor and indoor activities. There are 2 operable carbon monoxide detectors within the facility LPA observed fire extinguishers with an inspection date of 2/19/2025. LPA observed First Aid Kits with the required materials including a thermometer. Furniture and equipment were observed to be maintained in good condition free of sharp, loose or pointed parts. Tables and chairs were observed to meet the needs of the children. There are cots and mats for napping maintained in safe condition. LPA advised applicant to label mats so that the sleeping side can be distinguished from the floor side, There is a sufficient quantity and variety of age-appropriate play equipment in good condition. There are cubbies and hooks for storage of children clothing and personal belongings. The classrooms have central air for heating and cooling. The classrooms were clean in good repair. Classrooms have adequate lighting. Windows were in good repair free of chipping paint, dirt, insects, or debris. LPA advised applicant to install trash cans with tight fitting lids. The facility is equipped with a cell phone that is used for facility use only. LPA reminded applicant that cell phone must remain at facility during operating hours. Disinfectants, cleaning solutions, and other items that are dangerous to children are stored in a storage closet next to the activity room inaccessible to children. All toilets and hand washing facilities are maintained in safe and sanitary operating conditions. Hand washing fixtures do not deliver hot water. The applicant shall be responsible for ensuring that children with obvious symptoms of illness are not accepted. The isolation area for Ill children will be located in the Directors office. The isolation area shall be equipped with a cot for ill children and shall be supervised by staff. The staff restroom is designated for children who are ill . The applicant was advised to develop and implement a written plan to record the administration of prescription and nonprescription medications, to inform the child's authorized representative daily when such medications have been given. Medication will be stored in the highest top cabinet located in the kitchen inaccessible to children. Medication that requires refrigeration will be stored in the refrigerator located in the kitchen inaccessible to children. The program provides breakfast lunch and snacks. The kitchen and food-preparation storage areas were observed to be clean and free of litter and rubbish. All foods or beverages that require storage at a temperature of 45 degrees will be stored in 2 refrigerators. Director stated the smaller refrigerator will be used for dairy food items only. The refrigerators are not equipped with a thermostats to regulate temperature control. Menus are in writing and are posted in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request. The required postings are posted in the preschool activity room for parents and visitors to review. Facility does not have a disaster and mass casualty plan of action. LPA advised applicant the plan shall be in writing and shall be readily available with contingency plans for action during fires, floods and earthquakes. The facility does not provide transportation. Firearms and other weapons shall not be allowed on or stored on the premises of a child care center. The Applicant stated there aren\342\200\231t any firearms, weapons or bodies of water on the premises. Smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). Applicant was reminded that Licensees shall reveal each child care center license number in all advertisements in accordance with Health and Safety Code Section 1596.861 LPA advised applicant of the following regarding the toddler Program: 101216.4(a)(1) Preschool Program with Toddler Component A child who is between 18 months and 36 months of age may participate in the toddler program with written permission from the child's authorized representative. No child in the toddler program shall be placed in the preschool program before the age of 30 months without written permission from the child's authorized representative . 101429 Responsibility for Providing Care and Supervision for Infants Sleeping infant(s) shall be directly observed by sight and sound at all times . Staff shall physically check on sleeping infant(s) every 15 minutes. Documentation shall be maintained in the infant\342\200\231s file and be available to the Department for review . Outdoor The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. Toddler and Preschool Play equipment will need to be evaluated for age-appropriateness. Areas around and under climbing equipment, swings, slides and other similar equipment has wood chips for cushioning to prevent injury from falls. The outdoor activity spaces were observed and maintained in good condition. LPA observed a domed climber equipment not securely anchored in the preschool outdoor activity area. There is adequate shade and benches for resting in the preschool outdoor activity area. The Toddler outdoor activity space has an adequate amount of shading and benches underneath the awning area. The toddler large play equipment is in need of repairing chipping paint. Applicant was advised to inspect toddler sandboxes daily to ensure areas are free of any hazardous foreign material. LPA did not observe any bodies of water. The director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Incidental Medical Services (IMS) policy was discussed. Entrance Checklist was provided to the applicant. LPA discussed the safe sleep regulations with [applicant, licensee, or facility representative] and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep , as an additional resource. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for and removing recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters, and other important information communication platforms. The following corrections are needed before licensure will be considered. Install safety latch for drawer or cabinet to store sharp items. Mass Casualty and Disaster Plan Thermometer to regulate refrigerator temperature Applicant will label cots to identify sleeping side from floor side Applicant will ensure all trash cans have lids. Applicant will remove metal climber located in the preschool area that is not securely anchored to the ground. Facility will remove storage container with hinges from outdoor activity space inaccessible to children. Remove swing from tree Applicant will Provide Proof of age appropriateness of Toddler and Preschool Play Apparatus or provide pictures that equipment is inaccessible to children. Power wash Preschool Play Apparatus. Provide copy of conditional use permit. Corrections are due by. An exit interview was conducted with plan of corrected items discussed. LPA provided Facility Representatives with appeal rights and a copy of this report. Final license determination will be made upon review by the Licensing Program Manager and corrections.
