What the inspector wrote

On 5/06/2026 at 12:15PM, Licensing Program Analyst (LPA) Seena Parsapour conducted an unannounced Case Management – Other inspection, and met with Licensee Nayani Walawedurage. LPA explained the nature & purpose of the inspection to Licensee, which is to conduct a Confirmation of Removal (COR) for Crismy Wijamunige and to ensure Crismy Wijamunige is not present at the facility. At the time of the inspection, LPA observed eleven (11) children under the care & supervision of Licensee and one (1) assistant (cleared & associated). On 4/12/2026, the Department notified the facility that Crismy Wijamunige does not have a criminal record clearance and Crismy Wijamunige cannot work, reside, or be present at the facility unless a criminal record exemption is granted. During today’s inspection, LPA did not observe Crismy Wijamunige on site. Per Licensee, Crismy Wijamunige does not reside or work at the facility. The Department received a copy of the signed Removal Confirmation – Exemption Needed form (LIC300A) for Crismy Wijamunige on 4/16/2026. LPA informed licensee that they are required to notify parents of children currently enrolled that this individual has been removed from the home. Licensee is to have a copy of Addendum To Notification of Parent's Rights (LIC 995B) for each child in care. Effective today’s date, LPA has disassociated Crismy Wijamunige from the facility’s roster in the Guardian database. Verification of removal is complete. No deficiencies are being cited during today’s inspection. A Notice of Site Visit (LIC9213) was given to Licensee and must remain posted for thirty (30) consecutive days or a $100.00 civil penalty may be assessed. Copies of this report must be posted prominently for thirty (30) days in a visible location for the authorized representatives of children. Exit interview was conducted & report was reviewed with Licensee, Nayani Walawedurage. Appeal Rights (LIC 9058) were provided to Licensee. Licensee’s signature on this form acknowledges receipt of these rights.
