What the inspector wrote

On 9/22/2025 Licensing Program Analyst (LPA) Tyler Reyes arrived at the above licensed facility to conduct a Case Management-Deficiencies inspection to follow up on citations that were cited on 8/21/25. LPA met with Licensee Wolita, Childs and staff 1 (S1). LPA observed (3) children in care. LPA reviewed the following : Fire Extinguisher: LPA observed a fire extinguisher in the kitchen with a purchase receipt of 8/27/25. Family Child Care Homes are required to maintain a new or serviced fully charged fire extinguisher which is at least a 2A:10BC annually. LPA observed a room divider placed to prevent access to the wall heater. A recliner chair was positioned in front of the room divider. Poisons/Cleaning Products: LPA observed cleaning products in the kitchen to be inaccessible by key lock. LPA observed no personal hygiene products accessible in on-limit restroom. LPA observed the on-limit area of the backyard to be free of hazards. Staff Files: CPR/First Aid - American Red Cross Licensee Completed 9/6/25 and Valid 2 years. Licensee and S1 was informed that licensee and other personnel as specified shall complete training on preventive health practices, including pediatric carddiopulmonary resuscitation and pediatric cardiopulmonary resusciataion and pediatric first aid, pursuant to Health and Safety Code Section 1596.866 Children Files: Per Licensee Child 1 (C1) has been disenrolled. Per Licensee C1'parent plans to obtain medical exemption form for immunization. Licensee stated the documents will be forwarded to Department of Social Services upon receipt for review. Licensee understands that all children are required to provide proof of immunization for enrollment. LPA observed a completed LIC 627 for C3. Infants: Per Licensee there are no longer infants enrolled. LPA discussed infant safe sleep with Licensee. Cribs or play yards shall be free from all loose articles and objects. Facility should supervise infants while they are sleeping adhering to physical checks every (15-minute) and ensuring infants are sleeping on their back. Documentation of these checks shall be maintained in the infant\342\200\231s file and made available for Department review. Documentation shall include the date, infant\342\200\231s name, and the time of each (15-minute) check. The infant\342\200\231s authorized representative and emergency contact shall be notified if any signs of distress or labored breathing are observed. An individual Sleep Plan LIC 9227 shall be completed for each infant up to 12 months of age. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days consecutive days . Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview conducted and a copy of the report and appeal rights were provided to Licensee WOLITA. No deficiencies cited.
