What the inspector wrote

Licensing Program Analyst (LPA) Shandra Powell conducted an unannounced Required - 1 Year Inspection on 06/08/2023. A Risk Assessment for COVID-19 was completed. LPA met with Pamela Dale, Director. A copy of the Entrance Checklist for Child Care Center form (LIC 125) was provided to the Director upon entry. Director stated the facility is operating Monday-Friday 7:000am-5:00pm. Facility is licensed to serve 12 infants ages zero to 2 years old. The Director stated that she has 8 Infant children enrolled at this time. This is a Infant Center license who shares grounds with the Preschool/Toddler License. Per Director all staff present and working in the facility are fingerprint cleared and associated to the facility. LPA confirmed eligible clearance via Guardian Website during inspection. LPA observed 6 infants present with 2 teachers during inspection. Director, guided analyst on a tour of the facility inside and outside. LPA observed Facility License, Menus, Child Passenger restraint system poster (PUB269), Notification of Parents Rights (PUB 393), Emergency Disaster Plan (LIC 610) posted in a prominent, publicly accessible area at the facility (Parent Board). LPA reviewed the Disaster and Fire Drill Log (last fire drill conducted 04/2023) Sign in/out sheets, Daily Activity Schedule and Earthquake Preparedness (LIC 9148) during inspection(printed/completed during inspection). There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions are kept in the locked cabinet in the kitchen. LPA observed cabinet to have a child safety latch making the content and area inaccessible to the children in care. Teachers do the day to day cleaning. Medication is not being administer and facility does not have any children who need medication. Medications will be stored in a locked cabinet in the kitchen area. No poisons were observed inside of the facility. Furniture and equipment are in fair condition, free of sharp, loose or pointed parts. Playground equipment is in fair condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is gated and maintained in a safe condition and is free of hazards. Floors in the facility are clean and safe. Director stated rugs are cleaned monthly. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Dirty Diapers are discarded immediately. Families who bring in children\342\200\231s bottles and milk supply will label everything that is brought into facility. Water and baby food is provided by the facility. Older infants are served Breakfast, Lunch and Afternoon Snack. LPA observed all infant bottles to be labeled during inspection. Areas around high climbing equipment have cushioning material to absorb falls. The facility is free of flies, insects and rodents. Facility contracts with a pest control vendor to ensure facility stays pest fee. There is a working carbon monoxide and f acility has a pull fire alarm, fire extinguisher that was serviced on 05/30/2023, a Fire Prevention Inspection was completed May 2023 by th e Fire Department. LPA observed director test carbon monoxide during inspection. Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at off site activities. Director\342\200\231s CPR and Pediatric First Aid was taken on 11/23/2022 . Mandated Reporter Training has been completed by all staff members; Director\342\200\231s Mandated Reporter Training was taken on 09/10/2021. The name of the childcare center director or fully qualified teacher(s) designated to act in the director\342\200\231s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. During inspection LPA observed one infant had not been signed in by child's representative. This is a potential Health and Safety Risk to children in care. All children are under supervision, including visual supervision, of a teacher always. There is a ratio of one teacher supervising no more than four infants in care. LPA reviewed a total of 6 infant files during inspection. contact information for authorized representative and or relatives or others who can assume responsibility for the child, medical assessment, individual feeding plan, and Infant Needs and Services Plan. LPA reviewed all files for staff present during inspection and observed files were complete with qualification requirements, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training, CPR, LIC508, LIC9108 and LIC9052. LPA did not observe a completed LIC503 Health Screening Report for staff present during inspection. This is a potential Health and Safety Risk to children in care. Menus are posted at least one week in advance where an authorized representative can view them. Facility posted menu's a month in advance. The facility has sufficient age-appropriate furniture, and equipment including cribs, cot, changing table and feeding chairs. There is indoor and outdoor activity space for infants that is physically separate. LPA observed the outdoor space to be gated. Each crib, mat or cot is occupied by only one infant at time and cribs are kept free from all loose articles including blankets and pillows and there are no objects hanging above or attached to the crib. Infants are not swaddled while in care. Staff physically checks on sleeping infants every fifteen minutes and documents any signs of distress which includes but is not limited to flushed skin color, increase in body temperature, restlessness and labored breathing. LPA did not observed fifteen minute log for infants in care. Individual Infant Sleeping Plan is completed for only one infant during inspection. This poses a potential health and safety risk to children in care. Infants up to 12 months of age are placed on their backs for sleeping unless otherwise noted on the form LIC 9227 Individual Infant Sleeping Plan. LPA and facility representative discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. LPA gave copies of Safe Sleep Regulations and Safe Sleep Infant Pamphlets during inspection. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Director, Dale Appeal Rights were provided
