Inspector’s narrative

What the inspector wrote

8/22/25, 9/16/25, and 10/15/25, LPA Rivera observed bus with plates ending 96R2 with current license plates 3/2026, being utilized for school-age transportation. LPA did not observe preschool children coming off the bus. The buses do not require to have car seats due to, 5-point hardness that is built into the seats. Children stated, they do put on their seat belts. During the visits on 7/30/25, 8/22/25, 9/16/25, and 10/15/25, LPA Rivera did not observe the kitchen area to be unsanitary and has observed the meals and snacks on bowls covered with saran wrap and placed on carts and delivered to the assigned classrooms. Children stated the meals served are tacos, crackers, pasta, yogurt, bananas, applesauce, burritos, goldfish, and beans and different stuff every day. Staff stated have not observed chocking food being served to children ages 2. Staff stated personal phones are not being utilized for personal use. Phones are utilized to communicate among staff and iPad are utilized to communicate with parents, and upload photos. Staff and parents utilized the sproutabout app. Parents did not mentioned concerns with the quality of care. This agency has investigated the complaint alleging staff transported children in care without car seats, facility kitchen is unsanitary, facility transportation vehicles are not properly maintained, and staff did not provide proper food service to children in care At this time, it is determined that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore at this time the above allegations are unsubstantiated. No deficiency given at this time. Exit interview was conducted with director Melissa Edwards. The director was provided a copy of the appeal rights (LIC 9058) and signature on this form acknowledges receipt of these forms. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Based on interviews conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated. One type B deficiency is being issued today for regulation 101538.3 (b)-Indoor Activity Space for School-Age Children- (b) In combination programs, indoor activity space provided for school-age child care center children shall be physically separated from space provided for infant care and child care center children. Please see attached LIC 809-D for citation. Exit interview was conducted with director Melissa Edwards. The director was provided a copy of the appeal rights (LIC 9058) and signature on this form acknowledges receipt of these forms. This requirement is not met as evidenced by: Based on staff interviews, the facility did not comply with the section cited above in commingling school-age and preschool children in early AM and late PM. Staff were told by previews administration to commingle the children and staff were sent home.
