What the inspector wrote

On 8/04/2023, Licensing Program Analyst (LPA) Isabel Ortega conducted an unannounced subsequent complaint inspection to deliver findings on the above allegations. LPA met with Director Anne E. Grunbok, and toured the facility. Upon arrival LPA observed 28 children and six staff providing care and supervision. Palmdale Regional Office received confirmation case was assigned on 4/6/2023 based on an incident that occurred on 4/04/2023. This report was also, self reported by the facility. During the course of the investigation, Investigation Branch (IB) Investigator Laura Garcia conducted interviews and gathered documents relevant to the complaint allegation. A review of medical documentation and interviews with medical staff was not able to determine cause of injury. Staff was providing supervision. Based on evidence obtained and interviews conducted, the allegation of lack of supervision are deemed Unsubstantiated. A finding that the complaint is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged allegation occurred. An exit interview was conducted, a copy of this report, appeal rights and a notice of site visit were provided to facility.
