What the inspector wrote
On 11/16/2022 12:28PM, Licensing Program Analyst (LPA) Denise Miranda, conducted an unannounced Annual Required Inspection for the preschool License. LPA met with Alexandra Bessos, Business Manager and Shogher Melkonian, Administrator Admission Coordinator, facility does not have a qualify Director since 08/2022 and no Designated Responsibility of Facility on file. LPA toured the facility indoors and outdoors.
Days and hours of operation are Monday-Friday 8:00 AM \342\200\223 3:30 PM. LPA, observed 50 children in care with 8 staff provide care and supervision. Facility checklist (LIC 125) provided upon arrival to Director.
There are four classrooms designated to the preschool program. Facility is located on the C& E Meridian Armenian Evangelical School Campus.
There is no swimming pool or other bodies of water on the premises. Upon arrival, LPA observed the facility have one firearm security, per Shogher, Facility does not have firearms waiver on file. Per Alexandra and Shogher, firearm security service is been provide since 08/2021.
Disinfectants, cleaning solutions, medication and other hazardous items are observed accessible to the children in care. All poisons shall be kept in a locked storage area.
Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is
free of hazards.
The bathroom were inspected and observed the following: girls bathroom \342\200\223 3 sinks and 4 toilets and boys bathroom: 3 sinks, 2 toilets and 2 urinals, was noted to be clean and equipped, good flushing toilets, soap dispenser, paper towels.
Outdoor activity area is fenced all around and is supplied with age and size appropriate equipment. An adequate amount of cushioning material rubbish is in place under play equipment. Adequate shade is provided in outside play area. Drinking water is provided in the outdoor play area via water fountains.
Per Shogher and Alexandra, facility provided the morning and afternoon snacks and lunch. The food is prepared off-campus. LPA observed that facility has available snacks. LPA observed, sink, refrigerator, oven, and microwave. This area are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors.
Areas around high climbing equipment, slides have cushioning material to absorb falls, one small plastic slice equipment was observed that has broken pieces, per Shogher, that small plastic slide equipment will be remove from the outdoor. Facility has one functioning carbon monoxide detectors that meet statutory requirements and fire extinguisher.
Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption.
Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities.
The person who signs the child in/out of the facility shall use their full legal
signature and record the time of day. All children are under supervision, including visual supervision, of a teacher at all times. Facility shall maintain a ratio of one teacher supervising no more than 12 preschooler per full qualify teacher. LPA reviewed a sample of 5 children\342\200\231s files and observed files were not complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment.
Menus are posted at least one week in advance where an authorized representative can view them, however, facility missing to provide the amount and portions on their menus.
Criminal Record Statement:
Alexandra and Shorgher was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Incidental Medical Services (IMS) policy was discussed.
Per director no currently children needs medical services.

: California Child Care Workers; Mandated Training Requirement. Beginning January 1, 2018, all licensed providers, applicants, directors and employees must complete Mandated Reported Training prior to March 30, 2018 and renew training every two years at:

Nutrition Requirement:
Beginning January 1, 2016, AB 290 will require for each new license issued, at least one director or teacher at each child care center or family child care home to have at least
one hour of training in the importance of childhood nutrition
. This applies to anyone submitting a new application, relocating their facility, selling their facility or transferring their license. Please note this training cannot be completed online or by home study programs. The training must be taken from an Emergency Medical Services Authority (EMSA) approved training program OR an accredited college or university.
Staff Interview will be conduct on the next continuation annual Inspection
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018 requires all licensed Child Care Centers (CCC\342\200\231s) constructed before January 1,2010 to test their drinking water for lead contamination between January 1, 2020 and January 1, 2023, and then every five years after the date of the first test.

The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for
30 days
during the hours of operation after each site visit by a licensing representative.
Failure to maintain posting as required will result in a civil penalty of $100.00.
LPA explained that the deficiency is a Type A violation and requires immediate correction. Licensee was further informed of AB633 requirements. Licensee was provided with a copy of the LIC9224 and instructed to provide copies of this report to all parents of children currently enrolled and to obtain the parent's signature on form LIC9224, as acknowledgement that they received a copy of this report. Form LIC9224 is to be retained in the child's file. Licensee was further instructed to complete this same process for all children who enroll in the facility within 12 months of this report date.
LPA will continue this inspection without announcement. .
