What the inspector wrote

On 01/30/2026, Licensing Program Analyst (LPA) Justeene Tamayo met with Director Angelica Alvarez who guided LPA on a tour of the facility. The purpose of this visit was to conduct a Case Management - Incident Follow Up Inspection for an incident that occurred on 01/15/2026, and was reported on 01/16/2026. Upon arrival, LPA observed 12 preschool children in care with 7 staff present, and the Director. Summary of Incident: On 01/15/2026, Child #1 tripped and hit their forehead on a child size furniture. Facility contacted emergency services and contacted Parent #1 to inform parent #1 of the incident. Child #1 was provided with medical treatment by the paramedics. During the inspection, LPA Tamayo attempted to interview Child #1, Parent #1, and Assistant Teacher #1. LPA conducted follow up interviews with Lead Teacher #1, and Director. From interviews conducted, further follow up is needed. Facility was informed an unannounced case management incident inspection will be conducted at a later date. An exit interview was conducted, and a copy of this report was read and provided to Director Angelica Alvarez, along with a copy of appeal rights and Notice of Site Visit.
