On 02/05/2026, Licensing Program Analyst (LPA) Justeene Tamayo met with Director Angelica Alvarez who guided LPA on a tour of the facility. The purpose of this visit was to conduct a Case Management - Incident Follow Up Inspection for an incident that occurred on 01/15/2026, and was reported on 01/16/2026. Upon arrival, LPA observed 4 preschool children in care with 8 Teachers present, and the Director. Summary of Incident: On 01/15/2026, Child #1 tripped and hit their forehead on a child size furniture. Facility contacted emergency services and contacted Parent #1 to inform parent #1 of the incident. Child #1 was provided with medical treatment by the paramedics. During this inspection, LPA Tamayo interviewed Child #1, parent #1, and Teacher Assistant #1. Follow-up interviews were conducted with Lead Teacher #1 and the Director on 01/30/2026. Based on the interviews, the incident was determined to be accidental. Child #1 was walking quickly in the classroom, tripped, and struck their head on a small table, resulting in a minor laceration to the forehead. Teachers were present in the classroom, observed the incident, and did not have sufficient time to intervene before the fall occurred. First aid was immediately administered to Child #1, parent #1 was notified without delay, and emergency personnel responded to assist Child #1. Child #1 received stitches, and the stitches have been removed. Child #1 returned to school on 01/26/2026 and center obtained a return to school note from child #1 doctor. Based on the interviews conducted, no deficiencies will be cited. Center is notified to continue to report any unusual incidents to the Department within a timely manner. An exit interview was conducted, and a copy of this report was read and provided to Director Angelica Alvarez, along with a copy of appeal rights and Notice of Site Visit.
