What the inspector wrote

On 5/27/26, Licensing Program Analyst (LPA) Ranita Richmond conducted an unannounced 3year Inspection for the above-named Child Care Center. Center is licensed for 66 children (24 toddler and 42 preschool), ages 18 months to 5 years of age. LPA met with Supervisor Hilda Carbajal. Director Christina Morena arrived at approximately 1:45pm. LPA Richmond toured the facility indoors and outdoors and observed 71 children in care, being supervised and cared for appropriately by 12 fingerprint cleared staff members. Type A citation cited. See LIC 809D. Facility hours of operation are Monday – Friday, 8:00am – 5:30pm. Capacity and limitations as specified on the license are not being maintained. CCC has 59 preschool children in attendance, which is in excess of the 42 preschool children allowed per license. Type A citation cited. See LIC 809D. Director provided LPA with copies of previously submitted documents to complete a capacity change for the preschool and toddler components. LPA will submit documents to management for review. CCC uses Bright wheel app for sign in/ sign out daily. LPA Richmond advised Director of the importance of sign in/sign out using the full first and last name of the parent/authorized representative and time of checking the child in and out daily. During walkthrough at approximately 1:34pm LPA observed seven children in the toddler classroom using sleep sacks. Type B citation cited. See LIC 809D. LPA advised director that sleep sacks are prohibited due to being swaddling device. LPA advised director to send sleep sacks home with parents at the end of day. LPA observed that there is no infant sleep chart for toddler children enrolled under 24 months of age. Type B citation cited. See LIC 809D. LPA advised director that sleep charts are to be used to document sleep patterns of infants up to 24 months in age. Sleep charts shall be maintained in children’s files for review by the department. There are no swimming pools, ponds, or other bodies of water on the premises . There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible to children in care. LPA Richmond observed four fully charged 2A:10B:C fire extinguishers throughout the CCC. Fire extinguisher last serviced and or inspected May 2025. LPA observed last fire drill documented on 5/27/26. LPA Richmond observed working carbon monoxide and smoke detector duo in the classrooms. page 1 of 3 Toilets and hand-washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. LPA inspected the classrooms and observed age-appropriate toys, books and furnishings. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. Playground equipment is in safe condition, free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA observed a prepping area, with a sink and storage for foods. Foods and toxins or chemicals were stored separately. LPA did not observe any expired or contaminated foods. Solid waste storage containers have tight-fitting covers and are in good repair. LPA Richmond reviewed 12 children’s files and observed files to be in compliance including current contact information for authorized representatives and/or relatives who can assume responsibility for the child, immunization records, and authorization for medical treatment. LPA Richmond reviewed 6 personnel files. LPA reminded Director that CPR/First Aid is to be EMSA approved and both CPR/First Aid and Mandated Reporter training are to be renewed every 2 years. LPA reminded director that flu vaccination or declination is to be renewed yearly between August and December yearly. LPA observed 6 of 6 personnel files to have completed mandated reporter certification that has expired. Type B citation cited. See LIC 809D. LPA observed personnel files to have missing tb tests for 3 of 6 staff flies reviewed. Type B citation cited. See LIC 809D. CCC COMPLETED LEAD TESTING. Supervisor provided a copy of lead testing result to LPA for file. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. This facility does provide Incidental Medical Services – All medications are stored in the kitchenette area, in a top cabinet. All medications are individually stored in it’s original containers. Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. page 2 of 3 LPA Richmond reminded Director to access the Community Care Licensing website www.ccld.ca.gov for access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. LPA Ranita Richmond informed Director Christina Moreno that this report dated 5/27/26 documents two Type A citations which shall be posted for 30 consecutive days as there is/are immediate risks to the health, safety, or personal rights of children in care. Also, LPA Ranita Richmond informed the Director Christina Moreno to provide a copy of this licensing report dated 5/27/26 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Exit interview conducted, and report was reviewed with Director Christina Moreno. Appeal rights printed and reviewed with Director Christina Moreno. A Notice of Site Visit was provided and must remain posted for 30 days. page 3 of 3
