On 7/28/25 at 9:30AM, Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced,\302\240 Case management incident inspection. Upon arrival, LPA was guided on a tour of the facility by Assistant Director Beatric Gamora , to whom the purpose of the inspection was announced. Owners Maritza Levi and Eban Levi arrived around 10 am.\302\240Census:\302\240 There were eighty five children being supervised by twelve staff. Room 3C 8 children 1 staff Room 2A 17 children two staff Room 3A 6 children 2 staff Room 3B 11 children 1 Staff Room 2B 18 children 2 staff Room Pre-K A or room A 7 children 1 staff Room Pre-K B 18 children 3 staff. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence via phone on 7/9/25. The incident which occurred happened on 7/8/25 were child 1 was injured during water play by child 2 and their left armed was injured, later discovered the armed was fractured. During the inspection, LPA obtained a copy of the facility roster, facility sketch, copy of incident reported to Brightwheel, photos of splash pad, conducted interviews with 2 staff and 1 child. Due to time constraints, the investigation will be completed at a later time. Exit interview conducted and report was reviewed with Assistant Director Beatric Gamora .\302\240 A Notice of Site Visit was given and must remain posted for 30 days.
