What the inspector wrote

Licensing Program Analysts (LPA) Brittanee Cleveland conducted an unannounced case management visit on 03/12/2026 due to a self-reported incident that occurred at the facility. LPA arrived to the facility at 09:40AM and met with Debbie Heim, Director, who guided LPA on tour of facility. There were 124 children in care and 26 staff present upon arrival. The incident that occurred on 03/11/2026, was reported to the Department on 03/12/2026, via telephone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. This unusual incident was reported under the infant center\342\200\231s number by mistake. This incident occurred in the preschool. At the time of visit, LPA was able to collect and updated LIC624 Unusual Incident Report. Information reported to the Department stated that CPS visited facility to physically examine a child. Per director, a few hours later, LAPD arrived to the facility to pick up child and place them into protective custody. Director stated the CPS worker stated there was an investigation due to a call received regarding an older sibling. Per Director, Police stated that the director cannot call the parent. At the time of visit, LPA was able to conduct interviews with staff and observe the child. Child did fall asleep at the time of inspection. CPS worker stated they did not see anything for alarm on child. When police arrived to the facility, to keep this matter discreet, director was informed to get the child from play area and walk them to the police car. Director stated that this car was unmarked. Police and CPS worker left their cards for points of contact. Director stated that police attempted to call parent multiple times and did not receive an answer.--- page 2 Director stated the representatives that visited the facility did not disclose any allegations or investigation information. During the visit, Director received a call from parent. Parent stated that the older sibling said something, which was not disclosed with director, and CPS was called. Checking on the younger sibling was a part of routine protocol. Director stated the children were collected by police and taken to the station. Parent was able to retrieve children from the station after being given information on where the children were. Child returned to school on 03/12/2026. Staff and Director stated that child was acting like their normal self. Child has been monitored upon return. LPA advises the child should be monitored in case child has a reaction to the incident. Director stated though this may have been a mistake the child will be looked after with care. LPA advises if a direct issue happens with child to report it to the department. The Notice of Site Visit was given and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted and report was reviewed with the director, Debbie Heim. -- Page 2
