What the inspector wrote

On December 10, 2025, Licensing Program Analyst (LPA) Annelise Villa met with Director Sandra Clemons, who guided LPA on a tour of the facility. The purpose of this visit was to conduct a Case Management - Incident inspection regarding an incident that occurred on December 3, 2025. This Unusual Incident was self-reported on December 4, 2025. LPA discussed reporting requirements, pursuant to California Code of Regulations (CCR) Title 22, Section 101212(d), to ensure all reportable unusual incidents are reported within specified time frames. Upon arrival LPA observed 15, along with 5 staff including the director caring for them. Description of incident: On December 3, 2025, at approximately 12:58 pm, 2 daycare children were observed by director via video camera Staff #2 counted their classroom and went back inside once all children were accounted for. The children were left unattended from 12:41 pm to 12:58 pm. Director interviewed Staff #1 and confirmed no head count was conducted before taking the children inside, per facility policy. Director saw Child #1 and #2 alone on the playground and immediately called the nearest teacher to collect the children and return them to their classroom. Upon review of video footage, the children were left unattended outside for 17 minutes. Director interviewed all staff who were outside during playtime. Director dismissed Staff #1 effective 12/3/2025. Children were interviewed by staff and unharmed. Parent meetings were held on 12/5/2025 for viewing of the footage and to discuss any concerns. During this visit, LPA reviewed video footage, conducted interviews with staff, and collected relevant documents. As a result, the center was found in violation of Title 22, Section 101229(a)(1), No child(ren) shall be left without the supervision of a teacher at any time . This is a Type A deficiency, please see LIC 809-D for details. Continued on LIC 809-C Upon Receipt of a Type A deficiency, Director shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. Direcotr was advised LIC 9224 shall be kept in children's file. The Notice of Site Visit was provided, and LPA observed posting. Licensee is advised it must remain posted for 30 days.\302\240 An exit interview was conducted, A copy of this report and Appeal Rights were discussed and provided.
