What the inspector wrote

During today’s visit, LPA addressed the allegation that Licensee is not ensuring a safe environment for day care children in care. Based on LPA’s observation of the facility, where there is ongoing construction taking place on the grounds of the Church, where the facility shares the grounds with the St. Ephraim Syrian Orthodox Church. LPA observed there were construction workers, pallet of supplies, buckets, paint canisters, ladders, and other hazardous items near the walk way of the classrooms. During interviews with Staff #1, Staff #2, and Staff #3, they disclosed that construction has been happening for about 2 months and LPA observed hazardous items readily accessible to children in care (photos taken). The preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 Chapter 1, 101238 (c) Building and Grounds cited on the attached deficiencies page. As a result of today's visit, a Type A Deficiency was issued for regulation 101238 (c) Building and Grounds and a Plan of Correction is due 11/22/2025. Also, LPA Calvillo informed the facility representative to provide a copy of this licensing report dated 11/21/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days, exit interview conducted and report was reviewed with Tiffany Chapon, Director.

This requirement was not being met as evidenced by: Staf #1, Staff #2, Staff #3 disclosed that construction has been ongoing for 2 months and LPA during the tour of the facilit observed contruction activity near the facility classrooms. Which poses an immediate health, safety, and personal rights risk to children in care.
