Follow-up on corrections·November 21, 2025

On 11/21/2025, Licensing Program Analyst (LPA) Elicia Calvillo conducted a Case Management -Deficiencies Visit visit . LPA met with Tiffany Chapan, Director who guided LPA a tour of the inside and outside of the facility. There were 22 children and 2 staff upon arrival. During today's visit, LPA conducted a record review in that Tiffany Chapon, Director has been employed at the facility since 02/10/2025 and does not have a Criminal Record Clearance or exemption completed and/or associated to the facility. As a result of today's visit, a Type A Deficiency was issued for regulation 101170(e)(1) Criminal Record Clearance, a Plan of Correction is due 11/24/2025, and a Civil Penalty is assessed for $500.00. LPA informed facility representative Tiffany Chapon that this report dated 11/25/2025 document(s) one (1) Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Calvillo informed the facility representative to provide a copy of this licensing report dated 11/21/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
