What the inspector wrote

Licensing Program Analyst (LPA) Tatiana Bickham conducted an unannounced Proof of Correction (POC) inspection to the above facility on 4/1/2025.\302\240 LPA arrived at the facility at 2:20 PM, identified self and met with Fedrica Fernando, Licensee who guided LPA on a tour of the facility. There were 6 day- care children present during today\342\200\231s inspection. During the case management visit conducted on 3/17/25, the following deficiencies were issued: Type B- 1597.543- Carbon Monoxide Detector- LPA observed two carbon monoxide detectors. Type B- 102417(g)(9)(A ) - Fire and Safety Drill- LPA observed a fire and safety drill conducted on 3/19/25. Type B- 1596.8662(b)(1) - Mandated Reporter Training- LPA was provided a copy of the mandated reporter training for Licensee and staff. Type B- 102417(a)- Licensee shall be present during operating hours. Licensee submitted a declaration to LPA Bickham regarding not leaving staff in charge if she will be out of town and not being absent for more than 20% of operating hours. Type A- 1596.871(c)(1)(A)- LPA amended previous report and took off Type A deficiency for uncleared adult. LPA observed the staff cleared and associated to the facility under their previous name at the time of the visit. The deficiencies noted above have all been cleared and no further action is needed. The Notice of Site Visit was provided and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Licensee Fedrica Fernando, and Appeals Rights provided.
