What the inspector wrote

On May 29, 2026, Licensing Program Analyst (LPA) Annelise Villa met with Sara Pizinger, who guided LPA on a tour of the facility. The purpose of this visit was to conduct a Case Management - Incident inspection regarding an incident that occurred on May 27, 2026. This Unusual Incident was self-reported. LPA discussed reporting requirements, pursuant to California Code of Regulations (CCR) Title 22, Section 102416.2 , to ensure all reportable unusual incidents are reported within specified time frames. Upon arrival LPA observed 14 preschool age children and 3 staff including the director caring for them. Description of incident #1: On May 27, 2026, daycare children were playing outside, supervised by 2 staff. Child #1 was playing on a play structure ladder when Child #2 suddenly pulled Child #1, causing Child #1 to fall. Child #1 hit their head. Child #1 lost consciousness and was provided first aid by facility staff. Due to Child #1 regaining consciousness and was alert and awake within seconds, emergency medical services were not called. Parents were immediately notified and arrived within 10 minutes. Child #1 was transported to Antelope Valley Medical Center by parents to be evaluated. Child #1 sustained a concussion. The facility treated Child #1’s injury with cool water and contacted parent immediately. Child #2 did not sustain any injury. Interview revealed Staff #1 was approximately 4 feet away from Child #1 and Child #2, however, there was no time to prevent the incident from occurring. No deficiencies were found as a result of the incident. As a precaution, facility has decided to close the area of the playground to prevent similar incidents from occurring. Exit interview conducted, and report was reviewed with the licensee. A notice of site visit was given and must remain posted for 30 days. No citation was given today as a result of this inspection.
