What the inspector wrote

On 9/12/2025, Licensing Program Analyst (LPA) Amelia Morales conducted an unannounced case management inspection due to an incident which occurred on 08/01/2025. The written report was mailed and received to the Regional Office on 8/08/2025. LPA arrived at the facility and met with Director Georgina Garcia-Figueroa, who guided LPA on a tour of the facility. There were 6 infants in care during visit, supervised by five staff. The incident, which occurred, Child #1 bumped head and received a staple on injury. Child #1 was present during the visit, LPA Morales observed Child #1 run outside the playground. . During staff interviews LPA was informed that Staff #3 had active supervision of the child who obtained an injury. Staff #3 was in arms length when the incident occurred. LPA Morales inspected where the incident occurred. LPA Morales took photos of where the incident occurred. LPA Morales observed where the shelf is, there have been newly installed corner protectors. LPA Morales was informed by Staff #2, that Child #1 "tripped" " over there feet while getting up, and fell sideways. Staff #3 informed LPA Morales that Child #1 was trying to go towards a cushions toy that has mirrors. Staff #3 explained that they checked on Child #1, and noticed Child #1, was bleeding on the back of Childs #1 head. Staff #2 informed LPA that Staff #2, used paper towels to clean the area, was given an ice pack, and parents were notified of the incident. Page 1 of 2 LPA advised when submitting Unusual Incident Reports, to place the correct facility number on the LIC 624. LPA advised to place facility each facility number in each classroom, so there is no confusion. During the inspection, LPA obtained a copy of the children's roster, children's sign in/sign out sheet, conducted interviews with Staff, obtained medical records, took photos and made observations of where the incident occurred. Based on all information obtained on this date, and interviews conducted with staff, the incident appears to be an unusual incident. There were no deficiencies observed in regards to todays visit. Notice of Site was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Georgina Garcia-Figueroa. Page 2 of 2
