What the inspector wrote

On 03/19/2024 LPA Alemoh arrived at the above facility to conduct a case management visit in response to the receipt of an unusual incident report (UIR) from the facility. Upon arrival LPA observed 33 children with 3 staff, and two directors on site. At the time of this visit C1 was not present during today's inspection. The UIR was received by the licensing agency on 03/13/2024. LPA Alemoh arrived at the facility, met with the assistant director Melissa Rae, along with Director Pagie Martinez to review and discuss the submitted UIR. It was reported that on 03/08/2024 child #1 was left alone in the classroom. AD(assistant director), Melissa Rea and T2( teacher #2) observed C1 in a classroom which was supposed to be empty during lunch time. AD and T2 took C1 to the directors office to gather more information from C1 on why he was left in the classroom alone. Interviews revealed that T1 left C1 alone in the classroom for about five minutes due to a failed name to face roll call. The facility handled the situation by retraining all staff members on roll call, as well as name to face. The teacher who was in charge of C1 at time of this visit was not present. Based on interviews with staff it was revealed that it was revealed that T1 did not conduct the proper \342\200\234name to face\342\200\235 of the children thus leaving a child alone in the classroom for an period of time. The facility will be cited a Type B deficiency. See page 809D. Further information was gathered that all staff had completed the new CSR training's. Staff have signed off on their training that has been completed. Also along with newly updated CSR name to face checklists. A copy of this report was provided to the directors along with the appeal rights and the notice of site visit must be posted for 30 days.
