On 08/20/2024 at 09:30 AM, Licensing Program Analyst (LPA) Elicia Calvillo conducted an unannounced visit for Case Management -Other visit. LPA identified self and met with Thania Garcia, Director who guided LPA on a tour of the inside and outside of the facility. LPA observed 30 Children and 4 staff. During today's visit, LPA reviewed the Criminal Record Clearance for Staff #1 and Staff #2, issued citations, and assessed civil penalties. On 05/21/2024, LPA conducted a unannounced site visit to the facility and found that Staff #1 and Staff #2 were employed by the facility and did not have an Criminal Record Clearance associated to the facility. Facility representative did not request a transfer of a criminal record clearance for those individuals subject to a criminal record review. On 05/22/2024, Facility representative requested a transfer of the criminal record clearance for Staff #1 and Staff #2 for those individual subject to a criminal record review. The following deficiencies listed on the attached deficiencies page are being cited in accordance with California Code of Regulations Title 22, Division 12 Chapter 3, Section number 101170 (e) (2) Criminal Record Clearance and a civil penalty assessed for $100.00 for each staff. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. During this visit, the Director was provided with a copy of LIC 9224 Acknowledgement of Receipt of Licensing Reports Form and LIC 995A Notification of Parents’ Rights The Notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Thania Garcia, Director, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role.
