Inspector’s narrative

What the inspector wrote

On 06/27/2022, Child #1 came to school and parent had asked facility staff to keep the child away from water, especially the wound area. LPA reviewed the CSR dated 06/27/2022 and observed that there were 3 staff members supervising 12 children. LPA observed the facility is operating within the required teacher to child ratios. Facility accommodated the parent’s request and had a support staff provide one on one supervision with the child, however, while transitioning back inside the classroom, the child did not have the one on one staff anymore during the activity. The child ended up dipping the injured hand on the water table. Facility immediately informed Assistant Center Director and parent was notified right away. The parent arrived at the facility and child was taken to the doctor to get the child's bandage replaced. The child returned to the facility the next day and is still enrolled at the facility. Based on information obtained throughout the course of the investigation, the allegation that staff did not meet child’s needs is substantiated. A determination that the complaint is substantiated means that the allegation is valid because the preponderance of the evidence standard has been met. The facility is issued a Type B citation today, 09/22/2022. See LIC 9099-D for deficiency page. An exit interview was conducted and a copy of the report, appeal rights, and Notice of Site Visit were provided.
