What the inspector wrote

On 06/21/2022 Licensing Program Analyst (LPA) Doris Whitmore, conducted an unannounced Annual Required Inspection for the Family Child Care license. LPA met with Director Maryan Ghafari- Mobarhani toured the facility indoors and outdoors. Days and hours of operation are Monday \342\200\223 Friday 8:30a.m. to 5:30p.m. Total Children at the time of inspection were Five infants and one preschool child. LPA shared with the Director that she was out of ratio. Director called one of the Parents to pick up the child and was informed that it would be a Type a Violation. LPA only observed one staff which was the Director at the time of inspection. There is no swimming pool or other bodies of water on the premises . There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area. No poisons were observed during the inspection. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All foods/beverages capable of rapid spoiling are stored in covered containers at 45 degrees F or less. Solid waste storage containers have tight-fitting covers and are in good repair. All materials and surfaces accessible to children are toxic free. The facility is free of flies, insects and rodents. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. No individuals excluded by the Department are allowed to be present. Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. CPR and First Aid expiration date is 05/2024 The name of the childcare center director or fully qualified teacher(s) designated to act in the director\342\200\231s absence has been reported to the Department. All children are under supervision, including visual supervision, of a teacher at all times. LPA reviewed a sample of 6 children\342\200\231s files and observed files were complete except one file the immunization record was blank. LPA shared with the Director that she would need to document on the blue card the immunization's of the children. with contact information for authorized representative and or relatives or others who can assume responsibility for the child. LPA reviewed a sample of staff files and observed files were incomplete and were missing the mandated reporter LPA gave the website and shared with the Director to submit documentation of registration by Friday 06/24/2022 The Preventative Health and safety was last taken in 1996 the seven hour class without the Nutrition or Lead. LPA shared with the director and was given a flyer to register for the class and send documentation by Friday b 6/24/2022.and complete with documentation of meeting qualification requirements, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training. was not up to date. LPA shared with the Director that she needs another coy of her immunization record. LPA shared with the Director that the Mandated Reporter Training should be taken every two years. A overview was given to the Director on Safe Sleep and regulations reviewed as well. Incidental Medical Services (IMS) are not currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPA and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiency is being cited: (See next page 809D) Director was provided with a copy of appeal rights. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
