What the inspector wrote

On 07/29/2025 Program Analyst (LPA), Judy Laureano attempted to conduct an unannounced Annual 3 year Required Inspection at above mentioned facility. LPA arrived at approximately 12:15 pm and knocked at facility\342\200\231s door. LPA was able to hear a dog barking and children in the home. LPA knocked on window and announced herself, no one opened the door. LPA contacted Licensee via telephone and left a message requesting a call back. At approximately 12:35p.m LPA knocked on the window of the home and observed an individual open the blinds. LPA requested for the individual to open the door. LPA was greeted by M. Bernardez Lopez , assistant, who stated that Licensee was out. LPA discussed the purpose of the inspection and instructed M. Bernardez assistant to guide LPA on a tour of the home. Indi LPA inspected the home and observed 5 children present with M. Bernardez Lopez providing care and supervision. No citations were issued during today\342\200\231s inspection. A copy of this report and Notice of Site Visit was provided.
