Facility-requested visit·November 8, 2022

The Facility has submitted an application for increase of capacity from 24 to 68 with a toddler option up to 36 months, application was received on 8/26/2022. The facility has a total of 4 classrooms Classroom #1 and Room #2 are already included in the license. Facility is adding Room #3 and Room #4 for capacity increase and including a second play yard. Fire clearance was granted on 9/26/2022. The classrooms consist of cribs, changing tables with portable hand reachable sinks. LPA observed cubbies for the children's belongings. Furniture and equipment were inspected for age appropriateness and good repair. The telephone service, heating, lighting, and ventilation is adequate. There is filter drinking water in the classroom. There is first aid kit on the premises. LPA observed several age appropriate new toys and educational materials. A review of medication policy indicated that only prescribed medication is administered with parent's written permission. The medication dosage administered is documented on a log. There are cribs available for each child. There is a separate napping area separated by a partial wall in classroom #1 and #2. LPA observed cribs in Modular Room #2 the bedding is stored separately and properly. Children are inspected for illnesses as they arrive. There is a separate area for isolation in case a child becomes ill. The infant room will accommodate 57 children in the four classrooms. The following measurements are as follows: Room#1 and Room #2 Indoor Space : Modular Room 1 - 15 x 30 = 450. 450 '/. 35 square feet = 12 Modular Room 2 - 451 sq ft. '/. 35 = 12 Children (already included in the License) Classroom #3 (27 x 23) = 621 Sq. Ft divided 35 = 17 Children (classroom added) Classroom #4 (27 x 22) = 594 Sq. Ft divided 35 = 16 children (classroom added) The two outdoor yards will be utilized one play yard measured the following measurements: Outdoor Space (infant play yard): 34 x 64 = 2174. 2174 '/. 75 = 29 Second play yard: 78 x 75 =5,850 '/. 75 = 78 Total 57 Infants with toddler component, 12 children in each room #1 and room# 2 and 17 children in classroom #3 and 16 classroom #4. Went over safe sleep Provider information notice (PIN), LIC9227 infant safe sleep for up to one year of age (every three months) and infant log every 15 minutes up to the age of 24 months documentation form. The facility is in compliance per Title 22 regulations, an increase of capacity has been granted a total of 57 infants with toddler component. No deficiencies will be cited today 11/08/2022. An exit Interview was conducted, a copy of this Report and a Notice of Site visit was provided to the facility. Appeal rights were provided and discussed with facility.
