What the inspector wrote

On 01/16/25 Licensing Program Analyst (LPA) Ranita Richmond and Brittany Lovest arrived at the above named facility to conduct an unannounced case management deficiencies visit. LPA met with administrator, Kasa Adame. The above named facility is being issued a type b citation for responsibility for providing care and supervision based on LPA observation of outdoor play time. LPAs observed outdoor play from 9:30am- 11:07am. During outdoor play time LPA observed teacher arrive outdoors to the playground with two (2) children. Teacher takes a seat in a chair directly in front of the building and observes children. Approximately five (5) minutes later another teacher arrives outside with three (3) additional children. The two teachers, one sitting and one standing directly in front of the building stayed in close proximity to each other and verbally communicating with one another from 9:45am to 9:55am. At 9:55am on the outdoor playground, LPAs observed one teacher and five (5) children. LPAs observed teacher walk to outdoor storage shed area and retrieve a broom and sweep up the outdoor play yard while children continued to play on structures in the outdoor playground. At 10:04am LPAs observed two teachers and five children enter a door leading to the inside of the building. At approximately 10:07am LPAs observed one teacher exit the building to enter the outdoor play area with seven(7) children. Approximately two minutes later another teacher exits the building to enter the outdoor play area with five (5) children, for a total of 12 children and 2 staff. At 10:12am LPAs observed one teacher and twelve (12) children on the outdoor play area. At approximately 10:25am LPAs observed two teachers and 12 children on the outdoor play area. At 10:26am LPAs one teacher and twelve children on the outdoor playground. page 1 of 2 At 10:28am LPAs observed teacher open a door to the building with her back to children. At 10:35am LPA observed two teachers and twelve children on the outdoor playground. LPAs observed from 10:37am to 11:06am two teachers standing in front of the building communicating with each other. At 11:02 LPAs observed multiple children gathered at west end of the playground pulling, pushing, and swinging at each other. This lasted until 11:04am. At 11:05am children at the west end of the playground begin throwing objects at each other. 11:06am both teachers walked toward the east side of the playground. At 11:07am one teacher can be seen starting in the center of the playground and heading west, sweeping. At approximately 11:10am two teachers and twelve children enters the building. The above named facility is issued a type A citation for criminal records clearance due to S1 being employed for more than five (5) consecutive days at the facility prior to criminal records clearance completion. S1 was asked to leave facility to complete criminal records clearance. S1 will return to facility after criminal records clearance is approved. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, citations were cited today. See LIC 809D Exit interview conducted, and report was reviewed with Associate Director Kasa Adame. Appeal rights printed and reviewed with Associate Director Kasa Adame. A Notice of Site Visit was provided and must remain posted for 30 days. LPA Richmond informed associate director Kasa Adame that this report dated 1/16/25 documents 1(one) Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Richmond informed the associate director Kasa Adame to provide a copy of this licensing report dated 1/16/25 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. page 2 of 2

  • RESPONSIBILITY FOR PROVIDING CARE AND SUPERVISION

    101229 Responsibility for Providing Care and Supervision(a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision of a teacher... Supervision shall include visual observation. This requirement was not met as evidenced by: LPA Richmond and Lovest observation of outdoor play time activities. LPAs observed serval instances of lack of visual supervision.
