What the inspector wrote

On 05/07/2025 Licensing Program Analysts (LPA) Tyra Chavies conducted an unannounced Case Management visit. LPA Chavies met with director, Kasa Adame and toured the facility. While touring the facility LPA Chavies observed S# 1 changing C# 1 in front of the rest of the children in care. LPA Chavies advised S# 1 that this can be seen as a violation of the child's personal rights. S# 1 advised LPA that she will change children in restroom. On 05/07/2025 Licensing Program Analysts (LPA) Tyra Chavies conducted an unannounced Case Management visit. LPA Chavies met with director, Kasa Sanchez and toured the facility. While touring the facility LPA Chavies observed 8 children in care but only 7 were signed into the facility. LPA Chavies advised S# 1 of the situation, S# 1 replied " Oh, C# 1's guardian forgot to sign in." Per Title 22 Division 12 Chapter 1, the facility is being given a Type B citation and a technical Assistance. (See LIC 809-D and LIC 9102) An exit interview was conducted with lead teacher, Beatrice Sanchez and a copy issued. Notice of site visit was provided and must be posted for 30 days.
