On 07/15/2025, Licensing Program Analyst (LPA) Tyra Chavies  conducted an unannounced case management- incident visit to follow-up on a self- reported unusual Incident (LIC 624) reported to Community Care Licensing on 05/13/2025. Upon arrival, LPA met with Director, Kasa Adame.  LPA informed director about the purpose of the visit and toured the facility. LPA observed 17 children being supervised by 3 teachers. Incident details: Director noticed what looked like a small burn on C1 left hand and another one on the outer corner of his right palm, near his thumb. On 07/15/2025 LPA Chavies observed children in care, conducted interview director, children in care, and received personnel documents. Based on interviews conducted and review of personnel documentation, the facility and staff followed Title 22 regulations and the Health and Stay codes governing California. No deficiencies were cited. Exit interview was conducted and a copy of the report was provided to director, Kasa Adame. Notice of site visit was given and must remain posted for 30 days.
