What the inspector wrote

On 07/15/2025 Licensing Program Analyst (LPA) Tyra Chavies arrived at the facility to conduct a Plan of Correction visit and was met by Associate Director Kasa Adame. LPA observed  children in care being supervised and cared for by associate director and 3 fingerprint cleared staff. On 05/21/2025, Licensee was cited for the following: 1.(a)The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons. (3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation...This requirement was not met as evidenced by:  The facility failed to ensure the personal rights children in care were not violated. 2.  The licensee shall develop, maintain, and implement a written procedure to sign the child in/out of the child care center that shall, at a minimum, include the following: (1) The person who signs the child in/out shall use his/her full legal signature and shall record the time of day. (b) The person who brings the child to, and removes the child from, the center shall sign the child in/out. During today's visit  LPA Chavies observed the following: Licensee/Director  will hire someone or contact the facility's substitute, Arlene Estrada, who will assist when staff to child ratio exceeds regulations. Licensee/ Director has submitted this plan to LPA Chavies. LPA observed sign in/out sheet dated 07/15/2025, all children have been signed in. 2 of 2 Citations issued on 05/21/2025 has been cleared. An exit interview was conducted. A copy of this report, notice of site visit, deficiencies clearance letters were discussed and provided to Director Kasa Adame.
