What the inspector wrote

On 07/16/2025 Licensing Program Analysts (LPA) Tyra Chavies conducted an unannounced Case Management visit-Incident to follow-up on a self- reported unusual Incident (LIC 624) reported to Community Care Licensing on 05/29/25. LPA Chavies met with Director, Amber Bravo and toured the facility. There were  children being supervised by staff members. While touring the facility between 2:00-2:07 PM, LPA counted 15 children to 1 staff member with 5 awake children in the Preschool classroom, 16 children to 1 staff member with 6 awake children during the allocated nap time. Per Title 22 California Code of Regulations, the facility is being cited a Type A citation and a technical violation . (Please see LIC 809-D and LIC 9102). LPA Chavies, informed Director, Amber Bravo, that this report dated, 07/16/2025, with 1 Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care Also, LPA Chavies informed the director to provide a copy of this licensing report dated ,07/16/2025, that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. An exit interview was conducted, report was reviewed and provided to director Amber Bravo. Notice of site visit was provided and must be posted for 30 days.
