What the inspector wrote

On 9/14/2023 Licensing Program Analysts (LPA) Judy Laureano and Cristina Castellanos conducted an unannounced Case Management – Incident inspection related to a self-reported Unusual Incident Report (UIR) submitted on 8/4/2023. Upon arrival, LPAs met with Brenda Quintero, Assistant Director, and explained the purpose of the visit. LPAs observed 53 children in care supervised by 8 staff members and assistant director. According to the UIR, on 8/2/2023, child named on the report was grabbed by toddler teacher and place him on a chair. Child’s legs were bent upward and as the teacher pushed child in, child’s leg was caught between the table and child’s body. A parent dropping off a child in the classroom observed the incident and brought it to the attention of director. Director and Assistant director spoke with toddler classroom staff to discussed classroom management. Staff also received retraining around topics such as, but not limited to: communicating with families, how to manage specific challenging situation, consistency and positive guidance. Assistant director and director followed up with both parents involved. Facility followed all program guidelines and CCL’s reporting requirements. Staff involved is no longer at site. During today’s inspection, LPAs interviewed assistant director, Brenda Quintero and observed the toddler classroom where incident occurred. At this time, no further action is needed. An exit interview was conducted. A copy of this report (LIC 809) and Notice of Site Visit was provided.
