What the inspector wrote

On 11/08/2024, Licensing Program Analyst (LPA) Tyra Chavies  conducted an unannounced case management- incident visit to follow-up on a self- reported unusual Incident (LIC 624) reported to Community Care Licensing on 10/17/2024. Upon arrival, LPA met with Director, Derek Wienmann.  LPA informed director about the purpose of the visit and toured the facility. LPA observed 18 children being supervised by 4 teacher. Incident details: Director, Derek Wienmann, and P2 (Mother of C1) had a meeting regarding some general concerns with classroom management. In the meeting P2 disclosed that her son told her of two incidents where one of his friends was being hit by the a staff member. On 10/22/2024 LPA Chavies observed children in care, conducted interviews with director, assistant director, recent and current staff, C1, C2, C3 and C4, P1, P2 and P3 and received personnel documents. Based on interviews conducted and review of personnel documentation, once director was informed on the situation by parent, director followed proper procedures. No deficiencies were cited. Exit interview was conducted and a copy of the report was provided to director, Derek Wienmann . Notice of site visit was given and must remain posted for 30 days.
