What Happened
Required 3-year inspection of family childcare home found facility well-maintained with appropriate supervision of 7 children, but identified three deficiencies: uncovered heating element, expired mandated reporter training, and missing staff immunization records.

- Open face heater in living room covered only by screen, not fully enclosed (Type B citation)
- Licensee's mandated reporter training expired (last completed 09/30/22, needs renewal)
- Assistant has no mandated reporter certification on file
- Assistant immunization records missing (tdap, mmr, tb, flu)
