What the inspector wrote

On 1/7/26 , at 9:58AM, Licensing Program Analyst (LPA) Jeanine Lipsey conducted an unannounced Plan of Correction (POC) Visit. Upon arrival, LPA disclosed the purpose of the inspection and met with Licensee Angelica Diaz-Islas z, who guided the LPA on a tour of the facility. There were 5 day care children present during today’s inspection, of which 3 were infants. LPA also observed Staff 1 present at the time of this inspection. During the Required - 3 Year inspection conducted on 12/19/25, the following deficiencies were issued. Physical Plant - Type B: 102417(g)(1) - Fire extinguisher has not been serviced. Correction: Fire extinguisher was serviced on 12/23/25. Physical Plant - Type B: 102417(g) - Need plug covers in living room, outside playroom, kitchen, need safety gate near dog area, both entries to hallways, both storage areas, remove laundry detergent or gate or barrier before the washer and dryers, safety locks on kitchen cabinets and drawers, lock on outside cabinet. Correction: All plug covers were installed, both outside storage areas had barriers in front to prevent access, the laundry detergent was removed, LPA observed gates were ordered and due to arrive on 1/8/26 and LPA observed safety locks on kitchen cabinets and drawers, and locks on outside cabinets were installed. Facility Administration - Type B: 1596.8662(b)(1) - Licensee and staff missing mandated reporter training. Correction: Licensee and staff started Mandated reporter training in LPA’s presence and will send certificate when completed. Facility Administration - Type A: 102370(k) - Licensee Son Saul Islas and family member Samantha Resse has been visiting for 1 month and does not have fingerprint clearance. Correction: Licensee family member Samantha Resse and Son Saul Ialas now have fingerprint clearance as of 12/28/25. page 1 of 2 Due to the Type A received, Acknowledgement of Receipt of Licensing Report was present in the children's files. Records - Type B: 102418(g) - 1 child missing immunization's. Correction: LPA observed immunization in the child’s file. Records - Type B: 102425(j)(2)(D)(c) - 15 minute checks are not being documented. Correction: LPA observed 15-minute checks documentation. Physical Plant - Technical Violation: 102417(g)(9)(A) - Drills are conducted but not documented. Correction: LPA observed licensee conduct a fire drill with the children. The deficiencies noted above have all been cleared and no further action is needed. Exit interview conducted and report was reviewed with the Licensee Angelica Diaz-Islas. A notice of site visit was given and advised Licensee that it must remain posted for 30 days. page 2 of 2.
