What the inspector wrote

On 01/29/2026 , Licensing Program Analyst (LPA) Cristina Castellanos conducted an unannounced 3 Year Required Inspection for the above-mentioned facility. LPA Castellanos was greeted by staff member 1 (S1) and disclosed the purpose of the visit. LPA toured the facility both indoors and outdoors and observed a total of 30 children in care, being supervised and cared for by 4 fingerprint cleared staff members. At approximately 9:35 a.m., Dimuthu Ratnasekare, the designated director, arrived at the facility, followed shortly thereafter by Director Shirani Perera. The preschool program operates from 7:00am to 6:00pm. Additionally, an infant license operates at the same location. LPA observed the following teacher-to-child ratios: Classroom 1: 5 children being supervised by one (1) staff member Small Yard: 8 children being supervised by one (1) staff member Big Yard: 7 children being supervised by one (1) staff member Front Yard: 9 children being supervised by one (1) staff member At approximately 11:35am a child was dropped off at the facility. Capacity and limitations as specified on the license are being maintained during today\342\200\231s inspection. Continue During today\342\200\231s inspection, conducted between approximately 9:20 a.m. and 10:51 a.m., LPA Castellanos observed multiple instances of inadequate supervision. \302\247 At approximately 9:20 a.m. , LPA observed Child 1 (C1) using the children\342\200\231s designated restroom alone and without supervision. C1 then walked to the front yard independently and joined his classroom and Teacher S1. At no point during this time did S1 or any other staff member appear to be visually observing or actively supervising the child. \302\247 At approximately 10:13 a.m. and again at 10:51 a.m. , LPA observed Staff 4 (S4) standing inside the facility in the main hallway, facing away from the front yard, while her group of nine children remained in the front yard without supervision. At 10:51 a.m., S4 again stepped inside the facility for several seconds, leaving the children unsupervised. Children were not under supervision, including visual supervision, by a teacher at all times. The preschool program is not in compliance with Title 22 regulations section 101229(a)(1) Responsibility for Providing Care and Supervision - Type A. Parents sign their children in and out of the facility outside by the main entrance gate. LPA Castellanos advised licensee to ensure that parents sign using their full legal name. There are no swimming pools, ponds, or other bodies of water on the premises . There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions and other hazardous items are made inaccessible to the children in care. LPA Castellanos observed a total of one (1) fully charged fire extinguisher located between classrooms 1 and 2. The facility is equipped with a Pull Fire Alarm system located in classroom 3. LPA also observed a First Aid Kit in the children\342\200\231s designated restroom. Based on observation and interview, the licensee did not comply with the section 1596.954 Licensure Requirements by not having a carbon monoxide detector in the preschool classrooms, which poses a potential health, safety or personal rights risk to persons in care - Type B . All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. LPA Castellanos inspected the classroom and observed age-appropriate toys, books, and furnishings. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. Page 2 Playground equipment is in safe condition, free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Areas around high climbing equipment, swings and slides have cushioning material to absorb all falls. The facility does not provide meals to the children in care only snacks. Per the Licensee parents ensure they provide the meals for their children, which comply with dietary restrictions and allergies. LPA discussed the importance of maintaining a system where allergies and food restrictions are noted. LPA Castellanos observed the food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination, and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available to the children in care. Additionally, children bring in their own refillable water container. Prior to working or volunteering in a licensed childcare facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. A current CPR and Pediatric First Aid was observed for Dimuthu Ratnasekare, the designated director taken on 04/15/2025. LPA discussed the importance of making sure that all Pediatric CPR and Pediatric First Aid training is EMSA approved . LPA Castellanos reviewed 6 children\342\200\231s files and observed files to be in compliance including current contact information for authorized representatives and/or relatives who can assume responsibility for the child, immunization records, and authorization for medical treatment. Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 Page 3 Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). CCC COMPLETED TESTING AND HAS REMEDIATING LEAD EXCEEDANCES. LPA verified that the lead testing was completed in accordance with Written Directives outlined in PIN 21-21.1-CCP . - Collected on 09/05/2023 and was received on 09/12/2023. The facility provides Incidental Medical Services \342\200\223 IMS. Currently there are no children enrolled that require Incidental Medical Services. LPA reminded facility to ensure that all medication is stored with all necessary LIC forms. All prescription and nonprescription medication shall be centrally stored in accordance with the following requirements: medication shall be kept in a safe place inaccessible to the children in care, each container shall have an unaltered label. The Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. LPA reminded Licensee to access the Community Care Licensing website www.ccld.ca.gov for access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. Page 4 Per Title 22, Division 12, Chapter , of the California Code of Regulations, there were two (2) deficiencies cited during today\342\200\231s inspection \342\200\223 Type A and Type B. Additionally the Facility was also given Advisory Notes - Technical Assistance. Upon receipt of this report, Licensee shall post the Notice of Site Visit and any Licensing report documenting a Type \342\200\234A\342\200\235 deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgment of Receipt (LIC9224) form must be maintained in each child\342\200\231s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit. An exit interview was conducted, and Plan of Corrections were reviewed and developed with Licensee/Director Shirani Perera. A copy of this report and appeal rights were discussed and left with the Director, whose signature on this form confirms receipt of these documents. A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Page 5
