Routine inspection (multi-day)·February 19, 2025

On 02/19/2025 at 12:15PM, Licensing Program Analysts (LPA) Jeanine Lipsey conducted an Unannounced continued Required Inspection visit. LPA met with Director Eyde Mcallister, and toured the facility. Facility is currently licensed for a capacity of 181 children. LPA observed 143 children and 20 teachers present at the time of the visit. Orange room 3yrs: 6 children with 1 teacher Green room 2 yrs.: 16 children with 2 teachers Horizon #1 room 2 yrs. 7 children with 1 teachers Horizon #2 room 2 yrs. 4 children 1 teacher Frontier #1 room 4 yrs 15 with 2 teachers Frontier #2 room 4 yrs: 7 with 1 teacher Frontier #3 room 4 yrs: 9 with 1 teacher Frontier #4 room 4 yrs: 18 with 2 teachers Red room 2 yrs.: 10 children 2 teachers East wing 3 yrs: 15 children with 2 teachers Orange Curtain 8 yrs: 8 children 1 teacher Red Curtain 3 yrs: 10 children 2 teacher West Wing 3 yrs: 18 children 2 teachers Twenty Three Children\342\200\231s records were reviewed for the Immunizations Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents\342\200\231 Rights, LIC 613A Personal Rights, Admission Agreement, and LIC 701 Physician reports. Due to time constraints the visit will be concluded at a later date. All files were complete and in order. Exit interview conducted and report was reviewed with the Director Eyde Mcallister. A notice of site visit was given and must remain posted for 30 days.
