What the inspector wrote

On 06/12/2025, Licensing Program Analyst (LPA) Tyra Chavies conducted an unannounced case management visit. Upon arrival, LPA met with Assistant Director, Angela Palonis. LPA informed assistant director about the purpose of the visit and toured the facility. LPA observed  children being supervised by 13 teacher. On 06/05/2025 LPA Chavies cited the above facility a Type B citation for failure to protect the children in care personal rights. Due to computer issues, the deficiency page failed to print. LPA Chavies is at the facility to amend the previous report and present facility with the deficiency page. (LIC 809 D) An exit interview was conducted with Assistant Director, this report was read, and a copy of this report was given. Notice of site visit was provided and must be posted for 30 days.
