Facility-requested visit·January 8, 2025

An unannounced Case Management-licensee-initiated inspection was conducted by Licensing Program Analysts (LPAs) Amelia Morales and Silva Garibyan on this date in order to inspect and measure an additional classroom, which licensee wanted to add on to their existing license. Due to an emergency, LPA Garibyan left at 10:00 am and Licensing Program Manager Emiko Bell arrived at 11:00 am. Upon arrival at 08:11 am, LPAs were greeted by Head Teacher Pamela Trank. Senior Recreation Supervisor Lisa Crespo arrived at 11:00 am. Census was taken at 11:00 am, as children do not arrive until 09:00 am. Census: Room #150: there were 11 children with 3 staff; room #151: 15 children with 3 staff, and room #159: 14 children with 3 staff. In reviewing the associations, it was noted that Claudia Rodriguez was not associated to the facility; however, she has fingerprint clearance. A Type B citation is being issued. During the walk through of the facility, it was observed that the facility moved classrooms from room #140 to room #159. The previous Licensee did not notify the Department. However, fire clearance was received on 08/02/2024 for the rooms that are being in use which are Room #150 (NEW ROOM), Room #151, and Room #159. A citation has been issued due to the Department not being notified of the change of classrooms. Continued on 809-C On 1/08/2025, measurements were taken of the following rooms and outdoor area: Room #150 (NEW ROOM) total measurement : 710 sq. ft Room #159(NEW ROOM), total measurement: 689.88 sq. ft. Room #151, total measurement: 653. 23 sq. ft Outdoor area: MAIN PLAY AREA- Measurements: 3,109 sq. ft. BIKE AREA- Measurements: 2,342 sq. ft. BENCH AREA- Measurements: 273 sq. ft. Based on measurements taken today the maximum indoor capacity for Room #150 (NEW ROOM) is 20 children, Room #159(NEW ROOM) is 19 children and Room 151 is 18 children. The maximum outdoor capacity for the playground is for 77 children. However, the main playground has a capacity of 42 children, the bike area has a capacity of 31 children, and bench area has a capacity of 3 children. Notice of Site visit was given and must remain posted for 30 days. Exit interview was conducted with Recreation Services Manager Adrine Ovasapyan.
