On 10/10/2022 Licensing Program Analysts (LPA) Judy Laureano conducted unannounced Annual Required Inspection at 5130 Serrania Avenue, Woodland Hills, CA 91364. LPA met with Megan Eyer, facility director. LPA toured the facility indoors and outdoors and observed the following: Room 1- 8 children and 1 staff member Room 2- 10 children and 1 staff member Room 3- 10 children and 2 staff members Room 4- 8 children and 2 staff members Room 5- 13 children and 2 staff members Room 6- 11 children and 1 staff member Room 7- 13 children and 2 staff members Room 8- 9 children and 1 staff members During the inspection, 4 classrooms, rooms 5-8, were outside in the playground. Facility currently operates Monday through Friday LPAs toured and inspected the facility in accordance with the facility sketch. Teacher/child ratios are maintained, and adequate supervision is being provided during this visit. There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants and cleaning solutions are kept in each classroom, top cabinet with a safety latch, making the content inaccessible to the children in care. Director confirmed a cleaning crew comes every evening to clean the facility. Page 2 First Aid Kit bags were observed in each classroom and in the outdoor space. Medication is stored in each classroom unless medication needs to be refrigerated. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. LPA observed different shaded area in the outdoor space with age appropriate play equipment and toys. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. All toilets and handwashing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. Facility provides snacks and menus are posted and emailed to all parents. LPA\342\200\231s discussed with discussed the importance of maintaining menus for at least 30 days and noting any changes and/or substitution made. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors, filter water is provided via water pitcher to children in care and children bring in their refillable bottle. The facility is free of flies, insects and rodents, pest control services comes monthly. There is a working fire extinguisher, smoke detector, carbon monoxide detector and adequate heating and ventilation for safety and comfort. Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption. Upon notification from the Department, the licensee will comply and act immediately to terminate the employment of, remove from the facility or bar from entering the facility for any person it is deemed necessary while the Department considers granting or denying an exemption. Page 3 Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. Director\342\200\231s CPR and Pediatric First Aid expires 1/2023. Mandated Reporter Training is current for all staff, including Director. LPA reviewed 16 staff files and 1 out 16 files were incomplete. LPA issued an advisory and director agrees to ensure all documents are completed. LPA provided facility admin with a copy of LIC311A and LIC 125 to use as a guide to audit files and program. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. Facility uses an electronic sign in method for all parents and director prints sign in and out sheets daily to keep. All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care. LPA reviewed children\342\200\231s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. Facility representative, Meghan Eyer, was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Page 4 Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Incidental Medical Services (IMS) currently being provided. Licensee is aware that an IMS plan is required to be submitted to the licensing office if they provide any of these services. LPAs and facility representative discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
