Inspector’s narrative

What the inspector wrote

application that will allow them to sign the child in and out. The QR codes are located at the front gate and by the classrooms. Per Director if the parent/guardian forgets to sign the child in or out the teacher or the Director will sign them in or out. LPA reviewed sign in and out sheets for the last 2 months from June 1 - August 13,2025, LPA observed multiple parent/guardian signatures missing. LPA observed staff signing children in and out. LPA informed the Director the staff cannot sign the children in or out, it has to be the person dropping off or picking up the child. Based on the LPAs observations, the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated.  California Code of Regulations, Title 22 (code) is being cited on the attached LIC 9099D. Exit interview conducted with Director Forcila Funiestas. A copy of this report and appeal rights were provided. A "Notice of Site Visit" and copy of the report was issued. Notice of Site Visit must remain posted for 30 days. Failure to do so will result in a $100.00 civil penalty. Page 2.

This requirement was not met as evidenced by: LPA observation of time sheets from 6/1-8/13/25 with multiple parent/guardian signatures missing. This poses an potential healthy and safety risk to children in care.
