On 12/15/2023, Licensing Program Analyst, Adrian Risher conducted a case management inspection regarding an incident that occurred on 10/19/2023. LPA met with Director BillieJo LaBriola. LPA observed 61 children present with 24 staff. On 10/20/2023, ESCCRO received a call from the Early Education Manager regarding an Unusual Incident. The Early Education Manager reported a teacher(staff 2) informed her that a child was observed walking out in the garden area. Child 1 was taken to his classroom that he was assigned to. Staff 1 admitted that she did not do a head count when the children transitioned from the garden back to the classroom. Based on the investigation which included interviews with relevant parties and observations by the LPA, the preponderance of evidence to prove there was a Lack of Supervision violation has been met. This incident of Lack of Supervision has been Substantiated. Child 1 was left outside in the garden unsupervised after staff and children came back inside. The facility is being cited on the attached LIC 9099D page. Please refer to LIC421M for the Civil Penalty Assessment due to Lack of Supervision. LPA Risher informed Director BillieJo LaBriola that this report dated 12/15/2023 documents 1 Type A citation(s) which shall be posted for 30 consecutive days as there are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Risher informed the Director to provide a copy of this licensing report dated 12/15/2023 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC9224), or other written statement, must be placed in the child's file for verification. Exit interview was conducted and a copy of the report was provided. Appeal rights were reviewed and provided.
