On 6/18/24, Licensing Program Analyst (LPA) Ranita Richmond arrived at the above named Child Care Center to conduct an unannounced Case Management visit due to UIR received on 6/11/24. On 06/11/2024, LPA Ranita Richmond received a call from the Site Supervisor regarding an Unusual Incident. The Site Supervisor reported that during lunch while eating a child began choking on his food. A teacher performed Heimlich and the food became dislodged. Parents were notified immediately. Child remained at school for the remainder of the day. The site is a combo site with infant care. LPA toured facility preschool classrooms only inside and out and observed 27 children being cared for and supervised by 9 fingerprint cleared staff. Based on the investigation which included interviews with relevant parties and observations by the LPA, there is no preponderance of evidence to prove there was a Lack of Supervision or any other health and safety violations occured. Based on information obtained and interviews conducted, no further investigation is necessary. No citations were issued. Exit interview and copy of report provided to site supervisor Stephanie Thibodeaux. Notice of Site Visit, and Appeal Rights were provided to site supervisor Stephanie Thibodeaux.
