On 5/28/25 Licensing Program Analyst (LPA) Ranita Richmond and Chartrice Johnson arrived at the above named facility to conduct an unannounced case management incident visit due to unusual incident report (UIR) received via telephone to the case carrying LPA on 9/11/24. LPA was greeted by site supervisor, Stephanie Thibodeaux. Site Supervisor stated that she would be leaving for the day and that Center Coordinator Lisa Perez would assist today. LPA toured the facility both indoors and outdoors. LPA observed 27 children and 7 staff members providing care and supervision. Facility contacted the case carrying LPA on 9/11/2024 to report an unusual incident that occurred at the facility on 9/10/2024. Unusual incident report submitted to the El Segundo Child Care Regional Office states that while playing on play ground child tripped over his foot and hit his head on the ball statue causing red scratches and a little bruising to left side of his head in the front. Mother was called. Child remained at school until around 4:30pm.The day of the incident, site supervisor confirmed that 3 staff members were outside on the playground providing care and supervision to 9 children. During today’s inspection, LPA observed the outdoor space. LPA observed new zoning signs, including pictures of area where teachers will zone. The Unusual Incident Report investigation is complete. There were no citations issued during today’s inspection.An exit interview was conducted. A copy of this report and Notice of Site Visit was provided to Center Coordinator Lisa Perez.
