What the inspector wrote

***This inspection was conducted in English and Spanish***** Licensing Program Analysts (LPAs) Elicia Calvillo and Lilia Hernandez conducted an unannounced complaint inspection to the above facility on 08/22/2025. The purpose of the visit is to address deficiencies that were discovered during the course of an investigation conducted by the Department. Catalina Arrianga, Licensee arrived at the facility at approximately 12:00PM. LPAs arrived at the facility at 8:33AM. Upon arriving at the facility LPAs were greeted by Staff #1. LPAs introduced selves and were asked to wait by the locked gate on the side of the home where children enter the facility. As LPAs waited, LPA Hernandez observed that the children\342\200\231s toys were in front yard of the facility. LPA Hernandez also observed construction tools lined along the other side of the home and an open gate to access back yard (photos taken). Staff #1 returned after 10 minutes to open the gate that leads to entrance to the facility where children are in care. LPAs enter the facility along with a parent who had arrived to drop off their 2-year-old child at the facility. LPAs were then met by Catherine \342\200\234Collette\342\200\235 Arriaga, Assistant/Adult daughter of the Licensee. At approximately 9:09AM, LPAs observed a total of 15 children in care. Per Assistant, 3 of the 15 children in care were infants. The facility was not operating as specified in section 102416.5(a). This poses an immediate health, safety or personal rights risk to persons in care. LPAs were not able to confirm the ages of children in care because the facility did not have a completed LIC9040 Facility Roster for all children enrolled at the facility. The facility was not operating as specified in section 102417(g)(8). This poses an immediate health, safety or personal rights risk to persons in care. LPAs observed 1 infant and 1 preschool age child sleeping in play yards with blankets and loose-fitting sheets around the mattress. The facility was not operating as specified in section 102425(b). This poses an immediate health, safety or personal rights risk to persons in care. --- Page 1 of 2 LPA Hernandez continued the tour of the facility with the Assistant. LPA Hernandez observed 2 children restrained in highchairs with no food and no adult present. LPA Hernandez observed all off-limit areas identified on the facility sketch accessible to children. (photo taken) Per Assistant, off-limit areas were accessible because a deep cleaning of the home was being conducted. LPA Hernandez observed an off-limit bedroom with the door open and accessible. LPA Hernandez observed bottles of alcohol displayed and accessible on a side dresser in the bedroom (photos taken). During the tour of the back yard, LPA Hernandez observed the kitchen door opened and bottles of cleaning solutions accessible photos taken). LPA Hernandez also observed 3 buckets of stagnant water with insects in and above the water (photos taken). LPA Hernandez observed a charcoal grill, a barbeque grill accessible in the backyard where children were playing. LPA Hernandez also observed 2 propane tanks, one of which was connected to the barbeque grill (photos taken). The facility was not operating as specified in section 102417(g). This poses an immediate health, safety or personal rights risk to persons in care. LPA Hernandez observed alterations to the home. Per Assistant, the facility is being repainted, and the electrical system is being updated. Per Assistant, the detached activity room is also having the electrical system updated. LPA Calvillo did not find records of Licnesee reporting alteration to the home to the Department noted prior to alteration being done. The facility was not operating as specified in section 102416.3(a). This poses a potential health, safety or personal rights risk to persons in care. Based on observations by the LPAs, Assistant\342\200\231s self-disclosures, and Assistant\342\200\231s written statement, the deficiencies listed on the attached LIC 809D page are being cited. LPAs informed the Assistant that Type A citation(s) shall be posted for 30 consecutive days as there are immediate risk(s) to the health, safety, or personal rights of children in care. LPA Calvillo informed facility representative Catherine \342\200\234Collette\342\200\235 Arriaga that this report dated 08/22/2025 documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care Also, LPA Calvillo informed the facility representative to provide a copy of this licensing report dated 08/22/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for Page 2 of 3 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to mai ntain posting as required will result in a civil penalty of $100.00. Safety precautions shall include but not limited to:This requirement is not met as evidenced by: The licensee did not comply with the section cited above in that there were cleaning products, BBQ with propane tank, alcohol, doors open to off limits areas, readily accessible to children in care which poses an immediate health, safety, or personal rights risk to persons in care.

    102417(g)(8) Operation of Family Child Care Home(8) Each family child care home shall have a current roster of children as specified in Health and Safety Code Section 1596.841.This requirement is not met as evidenced by: The licensee did not comply with the section cited above in that current roster of children was not readily available to the Department upon request on 04/23/25, 06/12/25, and 08/13/25 which poses an immediate health, safety, or personal rights risk to persons in care.

    102425(b) Infant Safe Sleep (b) Cribs or play yards shall be free from all loose articles and objects.This requirement is not met as evidenced by: The licensee did not comply with the section cited above in that there were loose articles in the play yard while a child was asleep which poses an immediate health, safety, or personal rights risk to persons in care.

    102423 Personal Rights (a) Each child...shall have certain rights that shall not be waived or abridged by the licensee... These rights include...(4) To be free ...or other actions interference with eating, sleeping or toileting; or withholding ... or aids to physical functioning.This requirement is not met as evidenced by: The licensee did not comply with the section cited above in that Child #1 was left with dried feces down their leg from approimately 01:00PM until 03:30 pick up by parent which poses an immediate health, safety or personal rights risk to persons in care.
