What the inspector wrote

On Thursday, October 26, 2023 Licensing Program Analyst (LPA) Mayra Rivera conducted an unannounced case management inspection and met with Director Angelica Rodriguez. During this visit, LPA Rivera reviewed staff and children files and observed the files to be incomplete. LPA informed Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year, staff missing LIC 503 Health Screening Report, and staff missing AB 1207 Child Abuse Mandated Reporter Certificate and toddlers missing permission from the child's authorized representative for the placement of the child in the toddler program. During the visit LPA Rivera explained the citations and technical violations with director Angelica and director provided the plan of correction. Due date November 24, 2023. A notice of site visit was given and must remain posted for 30 days. An exit interview was conducted, during which this report and the Appeal Rights were discussed with Director Angelica Rodriguez.
